I, Mrs. Marcelina A. De La Cerna , allow my son/daughter, De La Cerna, Kenu A., a 4th


year student of BS Mechanical Engineering of this university to take IE 532 (
Technopreneurship), ME 527 (ME Laws, Contracts and Ethics), ICE 515 (Instrumentation and
Control Engineering), MEE 503 (Elective 3: Control Systems Engineering) and IE 530
(Engineering Management) subject this second semester. I am giving my full consent to take the
subject, including the additional payment for the tutorial, if this will be the best way to comply
with the requirement to finish the course this second semester (AY 2018-2019).
