Teacher:
Date of Screening:
X was screened on 10/21/16, using the Communication Appraisal Summary (CAS), an
informal 5-10 minute conversational speech sample; the Caroline Bowen Articulation
Screener; oral-motor examination; and the Fluharty Preschool Speech and Language
Screening Test-Second Edition (FLUHARTY-2).
X initiated verbal interaction, responded appropriately to questions, followed directions,
described actions, and repeated sentences. Oral structures and function were deemed
adequate for speech production.
In the area of Articulation, X exhibited multiple substitutions, including g/d; f/p; w/l; sh/ch;
b/v; distortion of /s/; and the phonological process of cluster reduction. He was not
stimulable for any of the misproduced speech sounds. His intelligibility is significantly
compromised by his misarticulations.
In the area of Language, he achieved a General Language Composite Quotient Score of
98 on the FLUHARTY-2, which indicates language understanding and use within normal
limits.
Based on the results of this screening, it is recommended that X be placed in Tier 2 of
Response to Intervention (RTI). Teacher and parent(s) will be given interventions/
suggestions/ examples/ materials to use in working with him to correct his
misarticulations. If sufficient progress is not indicated at the end of four weeks, he will
be moved to Tier 3, where he will receive an increase in the intensity and/or frequency of
intervention. At the end of 8-12 weeks, he will be rescreened to determine the need for
referral to the Speech-Language Impaired program.
