Non- Disclosure Agreement for Applicants
All information of the interviewee acquired by the company shall be treated as confidential in
nature and shall not be divulged by the company without the consent of the interviewee. In the same
manner, all information of the Company that the interviewee may acquire by reason of his/her
application shall only be for the purpose of evaluating the position offered and shall not be disclosed
without the Company’s consent.
The interviewee hereby authorize the company to collect, process and store all data his/her
data by reason of his/her application to the company. The purpose of collecting and storing such data is
to find the best client suitable/compatible to the applicant and if the applicant does not pass the
qualification of the clients, the company shall look for another suitable deployment for the applicant
based on the information/data collected. The interviewee also authorizes the Company to divulge the
data/information so collected to the clients wherein he/she is prospected to be deployed, the purpose
of which is to inform the clients of the qualification of the employee that would be posted on the latter’s
premise.
Storage of these data shall only be for five (5) years, after which, all data/information shall be
deleted/ removed from the Company’s storage, this applies to both electronic and paper-based data.
For clarification, data/information stated in this Agreement shall be those stated in their
resume/CV, or those that was provided in the Company’s Application Form.

Non-Disclosure Agreement (Nurses)
During my employment with Center for Health Services Inc. (CHSI), I acknowledge that I will be access to
patient information that is deemed sensitive and/or confidential.
I agree that:
a. I shall not share this information, material or documents with persons within or outside of
the Clinic or CHSI who are not authorized to have this information.
b. I shall not publish such information.
c. I shall not communicate such information without authority
d. I shall not remove any such information from the premises without permission.
e. I shall not use or disclose any such information for other than authorized official purposes.
f. Should I receive any such information, I will accept full responsibility to ensure the
confidentiality and safe-keeping of this information.
g. I shall take every reasonable step to prevent unauthorized parties from examining and/or
confidentiality and safe-keeping of this information.
I understand that these rules apply both during and after my employment with CHSI and that any
infringement by me of these rules may be grounds for the termination of my employment and/or legal
action.
