AFFIDAVIT

I, AILYN YANA JAPUS, Filipino, of legal age, and a resident of Purok


4, Sambulawan, El Salvador, Misamis Oriental do hereby depose and
state that:

1. I am the holder of a Voter’s ID issued by the Commission on


Elections and bearing VIN 4307-0080A-H1081AYJ20001-0 under
the name Ailyn Yana Japus.

2. After being issued my Voter’s ID, I recently noticed that the entry
“Date of Birth” bears the incorrect birthdate of August 10, 1981

3. As evidenced by my Certificate of Live Birth, the correct and


accurate date of my birth is August 15, 1981 and not August 10,
1981 as mistakenly printed in my Voter’s ID.

4. For all intents and purposes, I am the same Ailyn Yana Japus
identified in VIN 4307-0080A-H1081AYJ20001-0 only that my
birthdate entry is incorrect and should be changed from August
10, 1981 to August 15, 1981.

4. I am executing this affidavit to attest to the truth of the foregoing


and for whatever legal purpose it may serve.
