AFFIDAVIT OF ONE AND THE SAME PERSON


I, DFC, Filipino citizen, single, and a resident of Zone 7, Bangued, Abra, after
having been sworn in accordance with law aver that:

1. I am the child of FRANQUINAICY COLIS and LOURDES MILANA;

2. I have used the name DFC, in all of my records and dealings such as health
records, school records and others of similar nature;

3. My school record states my last name as COLIS. It is the ONLY document which
is in my possession to which my identity is based. As such, I have used the
last name “COLIS” instead of “MILANA” which is the last name appearing in
my Birth Certificate;

4. I am under the impression and belief that “COLIS” is my last name;

5. It was only during the time that I need to secure requirements for my
graduation that I discovered that my last name “MILANA” appeared in the
authenticated copy of my Certificate of Live Birth instead of my last name
“COLIS” which I am using since childhood;

6. I am executing this affidavit to attest that “DANNAH FLOR MILANA COLIS” and
“DANNAH FLOR CARIASO MILANA” as appearing in the copy of my certificate
of Live Birth are ONE AND THE SAME PERSON;

7. I am executing this affidavit to attest to the truthfulness and honesty of the


foregoing facts and for all intents and purposes it may serve.

Zambales, after having sworn to in accordance with law, hereby depose and say:

1. That we know personally FRANKLIN MIRADOR MOSELINA a.k.a.


DANILO MIRADOR MOSELINA who was born on December 27, 1961,
Filipino Citizen, of legal age and single;

2. That we know him to be using as his name DANILO MIRADOR


MOSELINA in all his private and public dealings instead of FRANKLIN
MIRADOR MOSELINA her true and correct name appearing on her Birth
Certificate;

3. That DANILO MIRADOR MOSELINA and FRANKLIN MIRADOR


MOSELINA is one and the same person;

4. This Affidavit is being executed to attest to the foregoing facts.
