Good day.

I would like to update you regarding the activities of Psychological Section that transpire from
February 1, 2017 up to date.

1. Held two meetings in February 7, 2017 and February 14, 2017 (see attached minutes)
2. Prepared draft of preliminary forms for your approval:
a. Intake Interview Form
b. Referral Form
c. Mental Status examination Form
d. Informed Consent Form
3. Administered psychological examinations (HTPT, DAPT, DAP-IQ, Reynolds Adaptable
Intelligence Test, BVMGT, Forer Sentence Completion Test, Hand Test, MMPI-2) among
7 referred patients. *The examinations materials were sponsored by A-Max
Psychological Services.
4. Prepared OJT Program
5. Proposed the Psychological Program Awareness Program on the whole month of March
2017 among the 6 dormitories.
6. Initiated to conduct an assessment procedure on the needs of our patients as bases for
psychological program. (see attached instrument).
7. Requisition of psychological examination (on-process).

The following are the up-coming activities:


1. February 21 and 22, 2017 Interview for a qualitative research from students of Cavite
State University among 7 patients. (see attached proposal)
2. February 22, 2017 LPU Cavite Educational Tour
3. Month of March, 2017 Proposed Psychological Awareness Program execution.
