TO THE FORMAL CHARGE

Respondent _______________ (Respondent), unto this Honorable Office,


respectfully states:

1. On ______________, respondent received a Memorandum from the


___________________________ requesting a sworn affidavit to explain

2. On ___________________, respondent submitted her sworn affidavit


containing an explanation for the recorded tardiness.

3. After preliminary investigation, on __________________, respondent


received a Formal Charge dated __________________ (the Formal Charge), where
this Honorable Office found that a prima facie case exists against respondent for

4. In said Formal Charge, respondent was given five (5) days from receipt of
said Formal Charge, or until ________________________, to submit her Answer under
oath, with attached affidavits and documentary evidence.

5. However, respondent would need additional time to gather the


documentary evidence and affidavits in support of her case. As such, respondent is
constrained to request this Honorable Office for an additional period of ten (10) days
