IN SERVICE LUMP SUM GRANT FOR RS: 5000000/- (RS: FIVE MILLION) IN LIEVE
OF PLOT.

With due respect I want to draw your kind attention to the following few lines for
sympathetic consideration and favorable action place.

Honored sir, my husband Mr. Roshan Din SA/30760 expired on 13-01-2017. It is intimated that
he has neither applied for allotment of plot in service nor he has been allotted any plot in the
past. Affidavit is attached herewith.

It is requested that I may please be paid Lump Sum Grant for Rs.5000000/-(Rs; Five Million) for
which I Allah remain grateful to you for ever and pray for your long life and prosperity.

Following documents are attached herewith for your few their necessary action.

1. Contingent bill in original


2. Photograph of widow
3. Photo copy of CNIC of widow
4. Photo copy of CNIC of decanted employee P.T.O
5. Copy of office order struck of strength
6. Copy of death certificate
7. Affidavit in original (widow hood certificate)
8. Copy of Nikah Nama.
