IMPORTANT INSTRUCTIONS
i)

Application Form will not be entertained if the same is not properly & duly filled in and
found missing therewith the following documents.

ii)

Two Photographs of Student attested by not below the rank of BPS-17.

iii) Attested photocopies of Fee Challan raised/demanded by the Educational Institution and
Boarding expenses (excluding meal charges)
iv) Income certificate from the employer i.e. Government/
service

v) Following documents must be attached in case of self-employed.


a. Bank Statement of Parents/Guardian for the last six (06) months.
b. Last 3 months utility bills of parents/guardian residence.
c. Last 03 months utility bills of parents/guardian business place in case of self
employed.
d. Tenancy agreement in case of living in rented house.
e. Registry of parents/guardian house, in case living in owned residence.

Income Tax return filed with FBR (In case of filer)

vi) Attested photocopies of :a. Domicile


b. Computerized N.I.C of Parents or Guardian and Student.
vii) Attested photocopies of all educational certificates

viii) Attested photocopy of the confirmation letter of University in which admission is granted.
ix) Application shall be considered on merit. Preference shall be given to students have
secured admissions in universities of high standing and low income of parents. Any
application which is not made on the prescribed form or is unsigned or does not contain
the required particulars and documents or reaches late will not be entertained.
