How to fill up the CDP Monitoring Form:


1. If the LGU has a CDP, indicate in Columns 3A-3C the period covered, SB Resolution, and date approved.
2. If the LGU has an outdated CDP and it is currently being updated, fill up columns 3D.1-3D.3.
2.1 Indicate under 3D.1, the phase/stage of the updating (i.e. Visioning, Situational Analysis, Goal Setting, Long List of PPAs, Prioritization, Investment Programming, For Approval)
2.2 Type 1 in the pertinent column specified under 3D.2 (Multiple attribution of financing is accepted):
Type 1 under 3D.2.1 if CDP updating is funded under PAMANA Pillar I;
Type 1 under 3D.2.2 if CDP updating is funded under DRR-CCA funded program of the Department ;
Type 1 under 3D.2.3 if CDP updating is funded by the European Union supported LGU Public Financial Management 2 Project (LGU-PFM2)and
Indicate under 3D.2.4, the funding source for those not funded by the abovementioned sources
2,3 Indicate under 3D.3 the agency/ies that are providing the technical support.
3. If the LGU does not have a CDP, type 1 in the pertinent column specified:
Type 1 under 4A if the CDP is already completed but has not yet been approved;
Type 1 under 4B if the local government unit has not yet ever formulated its CDP.
4. If the LGU is currently formulating a CDP for the first time, fill up columns 4C.1-4C.3.
4.1 Indicate under 4C.1, the phase/stage of the updating (i.e. Visioning, Situational Analysis, Goal Setting, Long List of PPAs, Prioritization, Investment Programming, For Approval)
4.2 Type 1 in the pertinent column specified under 4C.2 (Multiple attribution of financing is accepted):
Type 1 under 4C.2.1 if CDP updating is funded under PAMANA Pillar I;
Type 1 under 4C.2.2 if CDP updating is funded under DRR-CCA funded program of the Department ;
Type 1 under 4C.2.3 if CDP updating is funded by the European Union supported LGU Public Financial Management 2 Project (LGU-PFM2)and
Indicate under 4C.2.4, the funding source for those not funded by the abovementioned sources
4,3 Indicate under 4C.3 the agency/ies that are providing the technical support.
5. Please note that in the analysis, those that indicated ELA as CDP or CDP within the CLUP was classified as without CDP. Please indicate such important information together
with other details/information under Column 5 (Remarks) so that we can more accurately capture the status and current situation of CDP Compliance nationwide.
