Pathophysiology of Multiple Sclerosis

Multiple sclerosis is more common in women aged 20-40 of white race living in colder climates. Genetic and environmental factors like Epstein-Barr virus exposure, low vitamin D, and smoking increase risk. Sensitized T-cells cross the blood-brain barrier and attack myelin sheaths by activating macrophages and releasing cytokines, causing inflammation and destruction of the myelin sheath. This leads to plaques of sclerotic tissue on demyelinated axons and interruption of impulse transmission, resulting in various neurological manifestations depending on the affected neurons.

Sensitized T-cells cross the BBB

T-cells attach to myelin sheath receptors


Interferon gamma
activates macrophages

T-cells attach to macrophage


T-cells release cytokines

An inflammatory response is started


Macrophage release of tissue necrosis factor

Myelin is attacked

Destruction of the myelin sheath

Oligodendrocytes produce myelin in attempt to repair


the damaged myelin

T-cells, macrophages and interferon gamma works to


attack the myelin sheath as it was identified as non-self
