4. Use S.O.A.P. (Subjective, Objective, Assessment, Plan) notes to capture the
presenting issue:
Subjective: Student is dealing with a lot of family issues, and I think the root of
her grades going down is stress at home. We have focused on what makes her
happy/what she can do in the controlling environment.
Objective: Student was well-groomed during our session and wanted to talk to
me only about school. I gave her the option of no taping and being able to be
skip.
Assessment: The root of her presenting concern is psychological in nature; the
pressure she puts on herself to do well (possibly her parents too) is really high.
Also the stresses at home.
Plan:
o Specific: Check in with student about pressure on self, how she is
coping with life, and check ins about her EOC packet.
o Measureable: The next time we meet, asking about the above and
seeing if anything has changed.
o Attainable: Yes.
o Realistic: Very realistic and I want to follow up with her.
o Timely: Check in will occur next week.
a. Microlevel: I will be working one-on-one with Student over the remainder of

environment.

concerning her work, the EOC, and summer school.
7. List three things that you did well in the session with the client. List three things that
you can improve on in your session with the client.
I listened to her and let her tell me about schooling/grades. I was inquisitive about
her post high school. I celebrated her with her EOC accomplishment.
Gently pushing for more feeling words and seeing her emotional needs.
Highlighting the pressure she puts on herself. More open ended questions.
