Important Note: Check points and general advisory relating to the enrolment and insurance coverage of the applicant farmers have been mentioned in the last page of this
document. The applicant farmers are advised to go through the same.

Disclaimer: This document is only a farmer’s Premium acknowledgement receipt; coverage under crop insurance will be subject to the scrutiny of necessary documents and
applicability of relevant provisions as per the operational Guidelines of the PMFBY scheme.

Enrollment Check Points:

A. The applicant farmer is advised to cross-check the information entered by the CSC-VLE/bank/intermediary on receiving the acknowledgement receipt. Important details like land
details, bank account number, insured crop, insured area, the premium amount may be checked at the time of enrolment.

B. The authenticity of the receipt can also be verified by scanning the QR code printed above on page 1 of this acknowledgement receipt. The applicant farmer is advised to scan
the QR code and check the land profile details etc., reflected on scanning.

C. In case of any discrepancy, the applicant farmer is advised to report to the CSC Center/bank/intermediary immediately for rectification of any error observed.

General Advisory for the applicant farmers :

1. No processing fee is required to be paid by the farmer applicant for getting enrolled through any channel. Only farmers premium is required to be paid for enrolment through
CSC/Intermediary.

2. Incorrect information/documents or mismatch between information entered on the portal with details mentioned in the submitted documents could lead to the rejection of
applications.

3. Under the PMFBY guidelines, the applicant farmer can intimate within 72 hours after crop loss through Crop Insurance app, on toll-free numbers of the insurance company
mentioned on page 1 / bank/ State Government office about any localized calamity(hailstorm, landslide, inundation, cloud burst & natural fire due to lightning)/post-harvest losses
due to hailstorm, cyclone, cyclonic rains & unseasonal rains to harvested crops lying in the field in cut & spread / small bundled condition up to the maximum period of 14 days.
This shall be applicable to the notified crops and notified calamities only.

4. The claim amount is calculated on the basis of actual yield through Crop Cutting Experiments (CCE's) in the notified insurance area & Threshold Yield. While determining the
compensation under this scheme, the figures declared by any government department/institution regarding the percentage, drought, scarcity situation and flood damage will not
be considered.

5. The application farmer can track the status of his application through Crop Insurance App, which is an Android App and can be downloaded through Google Play store or the
