Hi,
Goldstone is renewing GMC (Medical Insurance Policy) coverage for the period 16th Sep 2021– 15th Sep 2022 This
communication is being sent out in line with this.
Action needed from your end –
† Please indicate the following details in table given below for yourself, spouse (as applicable) and children (as
applicable) – furnishing this information is mandatory for all employees receiving this email
† Note: Only a maximum of 2 children will be covered under the policy.
† You can enroll a maximum of 2 parents OR 2 in laws under ‘parental coverage’ as per the premium amounts
indicated below
† Only max of 2 life’s can be nominated under ‘parental policy’. It should either be parents OR in-laws. Cannot be a
combination of both
† Parental premium chargeable is indicated below. Based on nominations, equated amounts for total payable parents
premium will be deducted 3 months 2021 payroll
† Last date for sending self, spouse, children & parental nomination details is 10th April 2023 (before 12:00 PM
IST). No nominations or modifications will be accepted beyond this date
Parental Policy:
† We have three different Insurance coverage amounts offered to employees parents/in-laws as below
† Eligibility: Two Parent/Two In-laws (Combination of Parent and In-law is not acceptable)
† Premium Payment Terms: Premium will be deducted in three equated instalments along with the August,
September, October 2021 payroll
Note: You can opt for any of the 3 parental coverage limits according to the above table for a maximum of 2 parents or 2 in
laws. In case coverage is opted for both parents/in-laws, each individual will be covered separately according to the
coverage limit. For e.g. if an employee pays premium of INR 25,960 for both parents, both mother and father will be
individually covered for an insurance amount of INR 1,00,000 each. This is not a floater.
In case an employee exits the organization before the completion of policy, premium paid for parental insurance will not be
refunded.
Ensure to send your details in the below format on or before 10th April 2023 (before 12:00 PM IST). Indicate self,
spouse (if applicable), children (if applicable). Indicate parent/in-law details ONLY if you want to nominate them as per
above mentioned premium amounts.
In the ‘parental insurance coverage needed’ column indicate the coverage amounts requested for parents/in-laws (1, 2 or 3
lakhs)
