INTRODUCTION
A safety risk assessment is a systematic process to
identify, evaluate, and prioritize potential hazards or
risks that could cause harm to people, the
environment, or property.
Definition of terms
• HAZARD – anything that has the potential to cause harm.
• RISK- the likelihood that a person may be harmed or suffers
adverse health effects if exposed to a hazard.
• ACCIDENT : An unexpected , unplanned , sudden events
that results in injuries ,illnesses, damage to asset ,
environment .
• SAFETY : is a state or condition of being protected / free
from harm , danger, injury
HOW TO CONDUCT RISK ASSESSMENT
Safety Risk Assessment involve:
1. Identifying hazards: Recognizing potential sources of harm or
danger.
2. Assessing risks: Evaluating the likelihood and potential impact of
each hazard.
3. Prioritizing risks: Determining which risks require immediate
attention or mitigation.
4. Implementing controls: Putting measures in place to eliminate
or reduce risks.
5. Monitoring and reviewing: Regularly reviewing and updating the
risk assessment to ensure effectiveness.
STEP BY STEP GUIDE :
1. DEFINE SCOPE AND OBJECTIVES: Clearly outline the scope, goals,
and objectives of the risk assessment.

2. IDENTIFY RISKS: Brainstorm and list potential risks using techniques like:

• Brainstorming

• SWOT analysis : strengths ,weaknesses , opportunities ,threats .

• Review of historical data , Expert judgment

3 . PRIORITIZE RISKS: Rank risks based on their level, using techniques like
risk matrices or Pareto analysis.
4 . Record the findings and implement control : Collect relevant
data and information about each risk, including:
- Cause and effect
- Likelihood and probability
- Potential impact
- Existing controls
5 . Review and update Assessment : assess likelihood and
impact: Evaluate each risk's likelihood and potential impact using:
- Qualitative scales (High, Medium, Low)
- Quantitative scales (1-5 or 1-10)
- Probability and impact matrices. Determine risk level: Calculate
the overall risk level by combining likelihood and impact scores.

TYPES OF HSE RISK

1. Potential loss of life , asset , production , insurance.


2. Potential damage to health including injury and
sickness, environment , asset , reputation

All types usually manifest in serious financial and adverse


consequences
Risk Classification
Using the risk assessment matrix, risk is classified by three characters
made up as follows:

• The fist character is a measure of the likelihood of an undesired


event : a-e
• The second character is the consequence of severity that could occur
with that event : 1-5
• The third character shows to which consequence category the
assessment pertains : People(P),Assets(A), Environment(E) ,
Reputation (R)
The intersection of the chosen column with the chosen row is the risk
classification
Importance of Risk Assessment
• Protect employees and others from harm

• Comply with regulations and standards

• Reduce liability and financial losses

• Improve business continuity and reputation

• Enhance overall safety culture
