DECISION ON REQUEST FOR ACCESS TO A RECORD

(Section 25 of the Promotion of Access to Information Act, 2000 (Act No 2 of 2000))

Your request for access to a record was considered and the following decision has been taken:

A. Request for access to the record is granted

you whether you qualify for exemption from paying the relevant fee or not.

The access fee or reproduction fee payable is R35-00 . The fee may be paid at your nearest police station or financial
office of the South African Police Service (take this form with you when making such payment), or make an electronic
payment into the South African Police Service bank account of which the bank details are: ABSA Bank, Pretoria,
Branch Code 632005, Account Number: 4054522787, Cheque Account (the reference number on the top right side
of this form must be used as the relevant reference number). The receipt that you receive must be submitted to the
deputy information officer concerned as proof that you have paid the relevant fee. Only after the deputy information
officer has received the receipt, your request will be granted.

E. Refusal to confirm or deny the existence of the record

The South African Police Service refuses, in terms of section 39(3)of the Act, to confirm or deny the existence of
the record that was requested, for the following reason:

Note to all requesters:

You may, within 60 days, lodge an internal appeal against the access fee payable, the form or manner in which the
record will be made available, or against the refusal of the request, whichever is applicable to your request. If you want
to lodge an appeal, you must complete a Notice of Internal Appeal-form. A Notice of Internal Appeal-form may be
obtained from any police station. Such Notice must be submitted to the office of the deputy information officer
mentioned above. Within 180 days after being informed of the outcome of an internal appeal, you are entitled to lodge
an application to court for appropriate relief.
