2. My address as mentioned above falls within the Outer Ring Road (ORR) limits.

3. 1 am holding a valid driving license to drive an autorickshaw bearing.

4. License No. AP01220140009642 Valid up to: 19-05-2034

5. I hereby undertake that my date of birth, Aadhaar number, mobile number, and driving
license details are genuine and correct.

6. I hereby undertake that, my Drivers License is not involved in any disciplinary proceedings.

7. I hereby undertake that, I am producing all original uploaded documents at the time of
Registration of my autorickshaw.

8. I sincerely affirm and state that, all information provided during the application process at
the dealer level is correct and also declare that, I here by extend my consent for cancellation
of Registration if any aforementioned information is false.
