1. Reference: Memorandum from C, RCADD dated July 24, 2026 with


subject: Notice of Explanation.

2. In compliance with the directive to submit my written explanation, I


respectfully state the following:

3. Despite my continuous effort to coordinate with MORPPO PCADU and to


request timely information regarding any issues or incidents that may arise, I was not
able to fully accomplish my assigned monitoring responsibilities. I made repeated
attempts to establish communication and sought updates to ensure that official
content postings were properly supervised.

4. I acknowledge the importance of my duties and the responsibility entrusted


to me. I sincerely regret that, despite my coordination efforts, I was unable to meet
the expected outcome. I assure you that I remain committed to improving my
monitoring process and will take proactive measures to strengthen communication
channels to prevent similar lapses in the future.
