PRACTICAL REPORT

WEEK 1
ANTENATAL CLINIC
Today, I was posted to the Antenatal Clinic
(ANC) where I observed and participated in
the care of pregnant women attending
routine antenatal visits.
Antenatal care focuses on monitoring the
health of the mother and fetus throughout
pregnancy to ensure safe delivery and
prevent complications.
On arrival, patients were registered and
their case notes reviewed. Vital signs
including blood pressure, temperature,
pulse, respiratory rate, and weight were
checked and documented.
Abdominal examination was carried out to
assess fundal height, fetal lie, presentation,
and fetal heart rate.
Health education was given on nutrition,
personal hygiene, danger signs in
pregnancy, importance of regular clinic
attendance, malaria prevention, and birth
preparedness.
Routine medications such as iron and folic
acid supplements were administered e.g
vitamin c , paracetamol, ferrous , B complex
, calcium , folic acid. Tetanus toxoid
immunization was given when indicated.
I learned the importance of early booking,
proper documentation, respectful maternity
care, and effective communication with
pregnant women.
The experience enhanced my
understanding of maternal health
assessment and preventive care during
pregnancy.
WEEK 2
MATERNITY WARD
During my one-week clinical posting in the
Maternity Ward, I was involved in the care
of antenatal, intrapartum, and postnatal
patients under the supervision of doctors
and midwives. Throughout the week, vital
signs were monitored regularly, including
blood pressure, pulse, temperature, and
respiratory rate, and findings were properly
documented. I observed abdominal
examinations such as palpation and fundal
height assessment, as well as monitoring of
fetal heart rate.
I was exposed to the management of
women in labour, including observation of
labour progress using a partograph and
vaginal examinations performed by doctors
and midwives. I assisted in preparing
delivery materials and maintaining aseptic
techniques. I also observed the delivery
process and immediate care of the
newborn.
In addition, I participated in the care of
postnatal mothers by observing uterine
assessment, monitoring of lochia,
supporting breastfeeding, and ensuring
proper hygiene and infection prevention
practices. The posting improved my
understanding of maternal and newborn
care, labour management, and the
importance of teamwork in ensuring safe
outcomes for both mother and baby.
WEEK 3
FEMALE WARD
During my posting in the female ward, I
participated in several nursing activities and
gained practical experience in patient care.
I assisted in the administration of
intravenous fluids such as normal saline,
5% dextrose saline, and metronidazole. I
also administered injections including
paracetamol (PCM), Tandak, omeprazole,
and artesunate under proper supervision.
I observed and assisted with routine ward
procedures such as preparing and
administering medications, monitoring
patients, and maintaining aseptic
techniques during procedures. Through
these activities, I improved my practical
skills in drug preparation, IV administration,
and safe handling of ampoules and
syringes.
During the posting, I also learned the
importance of carefulness and safety when
handling sharp instruments.
Overall, the experience in the female ward
was very educative and helped me develop
more confidence in clinical practice while
improving my understanding of patient care
and ward management.
WEEK 4
CHEST CLINIC
During my rotation in the Chest Clinic, I was
exposed to a variety of respiratory cases,
including chronic cough, asthma, chronic
obstructive pulmonary disease (COPD),
pneumonia, tuberculosis, and interstitial
lung diseases. I participated in patient
history taking, physical examinations, and
the interpretation of investigations such as
chest X-rays, spirometry, sputum analysis,
and other relevant laboratory tests.
I observed and assisted in the management
of both acute and chronic respiratory
conditions, including pharmacological
therapy, inhaler techniques, and lifestyle
modifications. Emphasis was placed on
patient education, early diagnosis, and
follow-up care. I gained practical
experience in counseling patients on
adherence to treatment, recognizing
red-flag symptoms, and preventing
complications.
This rotation improved my clinical
reasoning, enhanced my understanding of
respiratory pathophysiology, and
strengthened my skills in performing
thorough respiratory examinations and
interpreting diagnostic results.
WEEK 5
INFANT WELFARE CLINIC
Today at the infant welfare clinic, I observed
and participated in various activities
focused on the care of infants and young
children. The clinic started with the
registration of mothers and their babies,
where basic information such as age,
weight, and health history were recorded.
Vital signs of the infants were taken,
including weight and temperature, to
monitor their growth and development. I
also observed how the healthcare workers
educated mothers on proper breastfeeding
techniques, nutrition, hygiene, and
immunization schedules.
Immunization was one of the major
activities carried out today. Vaccines were
administered to eligible infants according to
the national immunization schedule, and
mothers were properly informed about the
importance of completing their child’s
immunization.
Health talks were given on topics such as
exclusive breastfeeding, prevention of
common childhood illnesses, and the
importance of regular clinic visits. Mothers
were encouraged to ask questions, and
their concerns were addressed.
I assisted in simple tasks and observed
how the nurses handled both the babies
and their mothers with care and
professionalism. The environment was
clean and well-organized, ensuring the
safety of both staff and patients.
Overall, the experience was informative and
helped me understand the importance of
early childhood care, proper nutrition, and
preventive health services in promoting the
well-being of infants.
WEEK 6
PEDIATRIC CLINIC
During my posting at the Paediatric Ward, I
was exposed to the care and management
of infants, children, and adolescents with
various medical conditions. The posting
provided me with valuable experience in
observing the unique healthcare needs of
children and the importance of providing
family-centered care.
Throughout the posting, I participated in
taking and recording vital signs, including
temperature, pulse rate, respiratory rate,
and blood pressure. I observed and
assisted in the monitoring of patients'
conditions and gained experience in
maintaining accurate documentation.
I was exposed to the management of
common childhood illnesses and learned
the importance of early assessment, prompt
treatment, and continuous monitoring in
preventing complications. I also observed
the administration of medications and
intravenous fluids, while adhering to the
principles of safe and effective nursing care.
In addition, I learned the importance of
effective communication with both children
and their caregivers. I observed how
healthcare professionals provided health
education to parents regarding medication
adherence, nutrition, hygiene, and follow-up
care.
The posting enhanced my understanding of
infection prevention and control measures,
patient safety, and the need for compassion
and patience when caring for sick children.
It also improved my clinical skills,
confidence, and ability to work
collaboratively with other members of the
healthcare team.
Overall, my experience in the Paediatric
Ward was highly educational and
rewarding, as it broadened my knowledge
of paediatric nursing care and strengthened
my professional competence.

WEEK 8
MALE WARD
During my posting at the Male Ward, I was
exposed to the nursing care and
management of adult male patients with
various medical and surgical conditions.
The posting provided me with an
opportunity to observe and participate in
patient care while applying the principles of
nursing practice.
Throughout the posting, I assisted in taking
and recording vital signs, including
temperature, pulse rate, respiratory rate,
and blood pressure. I also participated in
routine nursing procedures and observed
the administration of medications as
prescribed by physicians.
I gained experience in monitoring patients'
conditions, documenting observations, and
reporting any abnormalities to the
appropriate healthcare personnel. I also
observed and assisted in procedures such
as wound dressing, bed making, personal
hygiene care, and the maintenance of a
clean and safe ward environment.
The posting enhanced my understanding of
patient-centered care and the importance of
effective communication with patients and
their relatives. I learned the value of
teamwork and collaboration among
healthcare professionals in ensuring quality
patient care.
Furthermore, I gained knowledge on
infection prevention and control measures,
proper documentation, and the ethical
principles guiding nursing practice. The
experience improved my clinical skills,
confidence, and ability to provide
compassionate care to patients.
Overall, my posting in the Male Ward was
educational and rewarding, as it broadened
my knowledge of nursing care and
strengthened my practical and professional
competence.
WEEK 9
ACCIDENT AND EMERGENCY WARD
During my posting at the Accident and
Emergency (A&E) ward, I was exposed to
the rapid assessment and management of
patients presenting with acute and
life-threatening conditions. The unit was
fast-paced, requiring prompt
decision-making, teamwork, and efficient
patient care.
Throughout my stay, I observed and
assisted in the management of various
emergency cases including road traffic
accidents, severe malaria, febrile illnesses,
injuries, and other urgent medical
conditions. I witnessed how patients were
triaged based on the severity of their
condition to ensure that critically ill patients
received immediate attention.
I participated in taking vital signs such as
temperature, pulse, respiration, and blood
pressure, and assisted in monitoring
patients’ conditions. I also observed
procedures such as intravenous (IV) line
insertion, administration of emergency
medications, wound dressing, and fluid
therapy. In some cases, I assisted in basic
nursing care under supervision.
I learned the procedures carried out after a
patient’s death, such as confirmation of
death by a doctor, proper documentation,
respectful handling and preparation of the
body, and transfer to the mortuary following
hospital guidelines. In addition, I gained
knowledge of postmortem changes,
including Algor mortis, Livor mortis, and
Rigor mortis, which involve body cooling,
blood settling, and muscle stiffening after
death.
I also learned that before administering
insulin to a diabetic patient, it is important to
assess the patient’s potassium level. This is
because insulin facilitates the movement of
potassium from the extracellular fluid into
the intracellular space, which can lead to a
reduction in serum potassium levels. If the
patient is already hypokalemic, insulin
administration may worsen the condition
and result in complications such as cardiac
arrhythmias.
This posting helped me understand the
importance of time management, effective
communication, and teamwork among
healthcare professionals in emergency
situations. I also learned how to remain
calm and composed while handling patients
in distress.
Furthermore, I gained knowledge on
infection prevention and control practices in
emergency settings, as well as the need for
proper documentation and patient
monitoring.
In conclusion, my experience in the A&E
ward was highly educational and impactful,
as it improved my clinical skills, boosted my
confidence, and prepared me to respond
effectively in emergency situations.
WEEK 10
DENTAL CLINIC
During my posting at the Dental Clinic, I
was exposed to the prevention, diagnosis,
and treatment of various oral and dental
conditions. The posting provided me with
valuable knowledge about oral health care
and the importance of maintaining good
dental hygiene.
Throughout the posting, I observed the
assessment and examination of patients
presenting with different dental problems,
including toothache, dental caries, gum
diseases, and other oral health conditions. I
learned the importance of obtaining a
thorough patient history and conducting
proper oral examinations before treatment.
I observed various dental procedures such
as tooth extraction, dental scaling and
polishing, and oral health education
sessions. I also assisted in preparing
patients for treatment and maintaining a
clean and safe clinical environment. In
addition, I learned the proper handling,
cleaning, and sterilization of dental
instruments to prevent infection and ensure
patient safety.
The posting enhanced my understanding of
infection prevention and control measures,
patient education, and the role of
healthcare professionals in promoting oral
health. I also learned the importance of
encouraging patients to practice good oral
hygiene through regular brushing, flossing,
healthy dietary habits, and routine dental
checkups.
Overall, my experience at the Dental Clinic
was educational and rewarding, as it
increased my knowledge of oral health
care, improved my communication skills
with patients, and broadened my
understanding of preventive and curative
dental services.
