Introduction
Background:
Pregnancy is the most crucial and delicate period for a woman. She needs to monitor
her and the fetus's health continuously. A lot of medical diagnoses need to be made.
One of them is the maternal and fetal heart rate and uterine contractions. The
information obtained from this surveillance during pregnancy is critical for determining
the fetus's health state and for prompt decision-making. In addition, arrhythmias are a
frequent consequence for pregnant women with cardiac problems. They require
constant observation. However, the usual methods of obtaining an ECG report are
difficult and expensive.
Furthermore, seeking a diagnosis at a hospital regularly is very difficult. We have
developed a method to create an ambulatory maternal and fetal ECG and EMG system
to address all these issues. The project aims to give expectant moms access to a
24-hour ECG and EMG monitoring system.
Although there are already certain products on the market, our goal is to create a
system that is inexpensive, simple to use and produces more accurate results. More
individuals will be able to profit from it thanks to these features. We are confident that
the technology will assist users in making important decisions, which could save lives
during delicate times.
Motivation:
A child is born in the USA every minute, and a newborn baby dies once every two
seconds. The current birth rate for Bangladesh in 2023 is 16.744 births per 1,000
people. The death rate in Bangladesh was 5.3 per 1,000 people. Although many
pregnancies end in miscarriage, some run over, these numbers show that our society is
concerned about future generations. To counter this trend, we are developing a better
system to save the lives of both expectant mothers and babies that can be born.
Problem Statement:
The most popular methods for monitoring the infant and finding any issues are
cardiotocography (CTG) and ultrasound. These devices employ ultrasonic technology,
are relatively large, are only offered in hospitals, and are not intended for continuous
usage. During normal pregnancies, the number of ultrasound and CTG screenings is
determined by the mother's age, the history of her health, and at least up until the
18th-week ultrasound scan, also known as the fetal anomaly ultrasound scan. However,
many parents find the comparatively long wait before the initial ultrasound scan and
confirmation of the embryonic heartbeat rather uncomfortable. Ultrasound exams are
scarce in many cultures. Moreover, there is no substantial proof that ultrasound can
harm the baby, but because it exposes the unborn child to sound waves, many medical
professionals advise using it cautiously. Hence, there is a need for a novel
non-ultrasound instrument that can detect the FHR with equal or higher accuracy
without endangering the fetus.
Design Specification:
Objective: Develop a system for fetal monitoring based on ECG and EMG signals to
differentiate between maternal and fetal heart rates.
Components: ESP32 microcontroller, AD8232 circuit board, ECG pads (electrodes),
Bandpass filter, notch filters, instrumentation amplifier, and power supply
Signal capture and processing:
● Electrodes will be placed on the maternal body to capture fetal, maternal, and
maternal EMG signals.
● Signals will be stored in a microcontroller and amplified to strengthen weak
signals.
● The Bandpass filter will remove unwanted noise and interference from amplified
signals.
● Microcontrollers will wirelessly send data to the monitoring device or system.
● Signals will be segmented into smaller epochs to analyze changes over time.
● R-peaks of the ECG signal will be extracted to calculate heart rates, and the
frequency content of the EMG signal will be used to detect uterine contractions.
● Segments will be classified based on the type of signal being analyzed.
● Spectrograms or line graphs will be used to visualize the results.
● Signal processing techniques will be used to extract more detailed information.
Result validation: Statistical analysis techniques will validate results and generate a
final report on maternal and fetal heart rates and uterine contractions.
