TASK TO PERFORM:

1 Verify the physician’s order: Check patient name, fluid type, volume, infusion rate, and duration against the Medication 2
Administration Record (MAR).
2 Perform hand hygiene: Wash hands thoroughly using the 6-step technique or use an alcohol-based rub. 2
3 Gather equipment: Collect the correct IV fluid , administration set (check drop factor), IV pole, alcohol swabs, tape, gloves, 2
and a sharpie/label ,iv cannula (correct gaze)
4 Check expiry dates: Inspect the fluid bag for clarity, leaks, or particles, and verify expiration dates on both the fluid and the 2
tubing package.
5 Perform the calculation: Calculate the drops per minute accurately before touching the patient. 2
6 Formula used: Drop Rate (gtt/min) = Total Volume (mL) ×Drop Factor (gtt/mL) / Time in Minutes 2
7 Calculation for this scenario: 1000 × 15÷8×60 = 15000÷480 = 31.25 =31gtt/min 2
8 Close the roller clamp on the IV administration tubing to prevent accidental spilling. 2
9 Remove the protective cap from the IV bag spike entry and the spike of the administration set using aseptic technique (do not 2
touch the sterile spike).
10 Spike the bag firmly, hang it on the IV pole, and squeeze the drip chamber until it is one-third to half full. 2
11 Slowly open the roller clamp to prime the line (fill it with fluid), ensuring all air bubbles are completely expelled from the 2
tubing. Close the clamp once primed.
12 Label the IV bag with the patient's name, date, time started, calculated rate, and your initials 2
13 Approach the patient, introduce yourself, and explain the procedure 2
14 Confirm patient identity using two identifiers (ask for name and DOB, and cross-check with the hospital wristband). 2
15 Assess the existing peripheral IV insertion site for any signs of phlebitis (redness, warmth, swelling) or infiltration (coolness, 2
swelling, blanching).
16 Don clean gloves. Clean the injection port/hub of the patient's peripheral cannula thoroughly with an alcohol swab for 15 2
seconds and let it dry.
17 Remove the cap from the IV tubing end and connect it securely to the cannula hub. 2
18 Slowly open the roller clamp and regulate the drop rate to 31 drops per minute by counting drops against a watch for one full 2
minute.
19 Secure the tubing safely with tape to avoid pulling on the insertion site. 2
20 Dispose of all waste materials properly, remove gloves, and perform hand hygiene. 2
21 Ensure the patient is comfortable and the call light is within reach. 2
22 Document the procedure: Record the date, time, type and volume of fluid, calculated drop rate, site condition, and patient 2
tolerance in the charts.
