SCENE 1 - Hospital Cafeteria
Narrator: "Good day, everyone. Our scenario involves a foreign body airway obstruction leading
to cardiac arrest. The setting is the hospital cafeteria, where a 72-year-old patient is having
lunch with his family. While eating, he suddenly experiences severe choking."
(Patient is eating with his family)
Althea: "Tay, kain pa po kayo."
Patient: "Salamat."
(Biglang mapapakapit ang pasyente sa leeg niya gamit ang dalawang kamay—ang universal
sign ng choking—at hindi na siya makapagsalita o makaubo.)
Jinella: "Tay? Okay lang po ba kayo?"
(Patient stands up, struggles desperately to breathe, and his lips begin to turn blue/cyanotic.)
Albert: (Sumisigaw) "Nurse! Nurse! Tulong! Nabibilaukan po siya!"
SCENE 2 - Initial Response
Narrator: "The primary nurse immediately responds to the scene, ensuring environment safety
before initiating care."
(Mabilis na lumapit si Silva habang nagsusuot ng gloves.)
Silva: "Excuse me po, paki-atras lang po nang konti para may hangin si tatay. Fe, paki-clear
muna 'tong area!"
Fe: "Copy. Sige po, paki-atras muna po."
Silva: (To the patient) "Sir, nabibilaukan po ba kayo? Nakakapagsalita po ba kayo?"
(Hindi makasagot ang pasyente, sobrang nangitim na ang mukha, at biglang bumagsak sa
sahig.)
SCENE 3 - Initial Assessment
Narrator: "As the patient loses consciousness, the nurse immediately transitions to the basic
life support protocol for an unconscious choking victim."
Silva: (Kneeling by the patient, tapping his shoulder) "Sir! Sir! Naririnig niyo po ba ako?" (Check
responsiveness) "No response. Unresponsive ang pasyente."
(Sabay na titingnan ni Silva ang paghinga at carotid pulse ng pasyente nang hindi hihigit sa 10
segundo.)
Silva: "Walang normal na paghinga. Walang carotid pulse. Confirmed cardiac arrest!"
SCENE 4 - Code Blue Activation
Silva: (Directing the team authoritively) "Jennifer, pa-activate ng Code Blue at tawagan agad
ang attending physician!"
"Ritchelle, simulan mo na agad ang high-quality chest compressions!"
"Samantha, ikaw sa airway—ihanda mo na ang bag-valve mask!"
"Dana, kunin mo ang crash cart at ang AED!”
"Nicole, paki-start nang mag-document ng lahat ng interventions at timestamps!"
Closed-Loop Communication:
Jennifer: "Copy! Activating Code Blue and calling the physician."
Ritchelle: "Copy! Start na ako ng chest compressions."
Samantha: "Copy! Opening the airway and prepping BVM."
Dana: "Copy! Fetching the crash cart at AED."
Nicole: "Copy! Starting documentation."
SCENE 5 - CPR
Narrator: "High-quality CPR is initiated immediately. Because this is a choking incident, the
airway provider will inspect the oral cavity before delivering breaths."
Ritchelle: (Nagbibilang nang malakas habang nagko-compress) "1, 2, 3... 20, 21, 22... 30!
Thirty compressions done. Ang lalim ay 2 inches, rate na 110 per minute, at may full chest
recoil."
Samantha: "Opening the airway using the head-tilt, chin-lift maneuver. Inspecting the mouth
kung nasaan ang nakabara..." (Looks inside the mouth) "May nakikitang foreign body sa
posterior pharynx!"
(Samantha performs a careful finger sweep or uses Magill forceps to remove the obstruction.)
Samantha: "Foreign body successfully removed. Clear na ang airway. Delivering first rescue
breath." (Delivers breath; chest rises) "May visible chest rise. Ibibigay na ang pangalawang
rescue breath." (Magbibigay ulit; aangat ang dibdib) "Airway cleared at ventilated. Resume
compressions!"
Nicole: "CPR started at [state current time]. Code Blue activated. Documented."
SCENE 6 - Doctor Arrives
Narrator: "The code physician arrives on scene. The primary nurse provides a concise SBAR
situation report while CPR continues uninterrupted."
( Doctor Jesabel arrives with an emergency kit.)
Doctor: "Status report, please."
Silva: "Doc, si John Paul po, 72-year-old male. Na-choke habang kumakain. Nag-cyanotic,
nag-collapse, at nag-cardiac arrest. On-going po ang high-quality CPR; natanggal na po ang
foreign body at clear na ang airway. Kararating lang din po ng AED."
Doctor: "Excellent summary. Silva, ikaw muna ang mag-monitor ng CPR para masigurong tama
ang lalim at bilis. Ritchelle, tuloy lang sa compressions hangga't hindi pa ready ang AED."
SCENE 7 - AED
(Dana arrives with crash cart at AED)
Dana: "Nandito na po ang crash cart and AED!"
Jennifer: "Ako na ang hahawak sa AED. Bubuksan ko na." (Mimics turning on device) "Ilalagay
na ang pads sa dibdib ng pasyente: upper right, lower left."
Jennifer: "Nag-a-analyze na ang AED! Lahat lumayo! Everybody clear!"
Ritchelle: "I'm clear!"
Samantha: "I'm clear!"
Doctor: "Clear."
Jennifer: "Everybody clear!"
Narrator: "The AED analyzes the patient’s cardiac.”
Jennifer: "Shock advised. Charging...Everyone stay clear! Delivering shock in 3, 2, 1... Shock
delivered!" ( Presses button)
Doctor: "Shock delivered. Resume chest compressions immediately! Huwag nang hintayin ang
prompt."
Ritchelle: "Resuming compressions!"
SCENE 8 - Compressor Change
Narrator: "To prevent fatigue and maintain high-quality compressions, the team switches
compressors every 2 minutes or after 5 cycles of CPR."
Doctor: "Dana, mag-establish ka na ng IV access at mag-ready ng 1mg ng Epinephrine. Fe,
tulungan mo siya sa IV line."
Dana/Fe: "Copy, establishing IV access.”
Ritchelle: (after continuing compression) "Naka-5 cycles na. Napapagod na ako."
Silva: "Jennifer, mag-ready ka nang pumalit bilang compressor. Move into position."
Jennifer: "Ready."
Silva: "Palit sa susunod na recoil! 3, 2, 1... Switch!"
(Jennifer seamlessly takes over chest compressions in under 5 seconds).
Nicole: "Compressor switched at [State current time]. Total of one shock delivered. IV line being
established."
SCENE 9 - Return of Spontaneous Circulation (ROSC)
Narrator: "Following the completion of the next full cycle, the doctor calls for a rhythm and pulse
check."
Doctor: "Stop compressions. Mag-rhythm at pulse check muna tayo."
(Jennifer pauses compressions. Doctor checks the carotid pulse).
Doctor: "May malakas at palpable na carotid pulse. Samantha, paki-check kung humihinga na
nang kusa."
Samantha: "Doc, kusang umaangat ang dibdib ng pasyente. Bumabalik na ang respiratory rate,
mababaw pero steady."
Silva: "May Return of Spontaneous Circulation (ROSC) na tayo!"
Nicole: "ROSC documented at [state the current time]. May pulse na."
Doctor: "Excellent work, team. Samantha, ituloy mo lang ang airway support at i-titrate ang
oxygen gamit ang BVM kung kailangan. Jennifer, paki-double check ang vital signs at kunan ng
blood pressure. Ihanda na natin ang pasyente para mailipat agad sa Intensive Care Unit (ICU)."
ENDING
Narrator: "Through prompt recognition, immediate activation of the Code Blue system, flawless
high-quality CPR, rapid airway clearance, and effective closed-loop teamwork, the patient
achieved a Return of Spontaneous Circulation. This concludes our simulation. Thank you."
