1. Capture the Complaint Accurately
The first record should preserve the customer's experience without forcing an early conclusion.
Information area Details
Product identity Brand, model, variant, serial/lot when available.
Purchase information Date, channel, order reference.
Use conditions How, when, and for how long the product was used.
Observed problem What the customer saw, heard, felt, or measured.
Outcome Injury, damage, failure, return, inconvenience.
Evidence Photos, video, unit, packaging, messages.
Use neutral language
Write "customer reported heat damage after 20 minutes" rather than "product overheated" until evidence confirms
the cause.

2. Assess Immediate Risk
Before searching for the root cause, decide whether other customers may be exposed.
● Is there a possible safety hazard?
● Could the issue affect the same lot or model?
● Should sales, shipment, or use be paused?
● Does the customer need urgent medical, technical, or replacement support?
● Are regulators, insurers, or senior management notification thresholds met?
Containment
Containment may include holding inventory, blocking a listing, contacting affected customers, or requesting return
of the unit.

3. Build an Evidence Timeline
A timeline separates known facts from assumptions and highlights missing information.
Timeline stage Evidence to collect
Before use Storage, condition, setup, instructions followed.
During use Duration, settings, environment, unusual events.
Failure moment Exact observation and immediate response.
After failure Photos, continued use, disposal, repair, return.
Business response Messages, replacement, inspection, escalation.
Preserve physical evidence
If the unit is returned, label it, photograph its condition, and avoid destructive testing until the investigation plan is
agreed.

4. Investigate Possible Causes
Consider product, process, user interaction, environment, and information factors.
Cause family Questions
Product design Specification, protection, tolerance, foreseeable misuse.
Manufacturing Materials, assembly, test records, lot variation.
Packaging/transport Damage, moisture, compression, wrong accessory.
Instructions Missing, unclear, or contradictory guidance.
Use conditions Setting, duration, covering, power source, maintenance.
Service response Advice that may have increased or reduced the outcome.
Do not stop at operator error
Ask why the error was possible and whether design, instructions, or controls should make it less likely.

5. Decide Corrective and Preventive Actions
Actions should address both the immediate case and the wider system when evidence supports it.
● Customer remedy: refund, replacement, repair, medical or damage process.
● Containment: sort, hold, stop shipment, retrieve units.
● Correction: fix the specific unit or batch.
● Corrective action: remove the confirmed root cause.
● Preventive action: reduce risk in related products or processes.
● Effectiveness check: verify the action worked over time.
Assign owners and dates
An action without an owner, deadline, and evidence of completion is only an intention.
