Hearing on Petition for Notarial Commission

Petitioner:
Good morning, Your Honor. I am Juan Dela Cruz, the petitioner in this case,
respectfully applying as a Notary Public for and in Quezon City. I am ready to
present the documentary requirements in support of my Petition.

May I proceed, Your Honor?

Court:
Proceed.

Petitioner:
Thank you, Your Honor.

I respectfully manifest that I have attached to my Petition the required documents,


marked as follows:

Annex “A” is my Community Tax Certificate No. ____________, dated


____________.

Annexes “B” and “C” are my Certificate of Membership in the Philippine Bar and
my Certification of Good Standing and No Pending Case issued by the Office of the
Bar Confidant of the Supreme Court.

Annexes “D,” “E,” “F,” and “G” are the certifications issued by the Integrated Bar
of the Philippines National Office and the appropriate IBP Chapter, stating that I am
a member in good standing and that I have no pending case.

Annexes “H” and “I” are proof of payment of my 2026 IBP membership dues,
consisting of the screenshot of the official receipt from the myIBP application and the
email payment confirmation.

Annex “J” is my Professional Tax Receipt for 2026.

Annex “K” is my NBI Clearance.

Annexes “L” and “M” are the photographs and sketch of my place of notarial work
located in Diliman, Quezon City.

Annexes “N,” “N-1,” and “N-2” are my three specimen signatures on bond paper.
