Short-Form State–Trait Anxiety Inventory (STAI-SF)

This questionnaire is designed to measure how you feel right now (State Anxiety) and how you
generally feel (Trait Anxiety).

Part A – State Anxiety (How you feel right now)


Please rate each statement:
1 = Not at all 2 = A little 3 = Moderately 4 = Very much

1. I feel calm. (R) 1 ■ 2 ■ 3 ■ 4 ■


2. I feel tense. 1 ■ 2 ■ 3 ■ 4 ■

Scoring Instructions:
Reverse score (R) items: 4→1, 3→2, 2→3, 1→4.
Add up each section separately:
• Part A (State): 5–20
• Part B (Trait): 5–20

Interpretation:
5–9 = Low anxiety
10–14 = Moderate anxiety
15–20 = High anxiety
