CVS-ICU Nursing Case Study: Hemodynamic Management &
Fluid Balance
➢Patient Profile: A 60 kg male patient is on Post-Operative Day 1 following CABG surgery. You are the
bedside nurse for the 08:00–16:00 shift. The patient is NPO (Nil per os)
➢Physician Orders :
➢Dopamine Infusion: * 08:00 – 10:00: Start at 4 mcg/kg/min.
➢10:00 – 16:00: Increase to 6 mcg/kg/min due to low mean arterial pressure.
➢Concentration: 200 mg Dopamine in 500 mL 5% Dextrose (Pre-mixed).
➢Maintenance Fluid: 1000 mL of 5% Dextrose at a rate of 100 mL/hr.
➢Diuretic Therapy: Continuous Furosemide infusion at 5 mg/hr.
➢Concentration: 100 mg Furosemide in 50 mL 0.9% Normal Saline.
➢Antibiotic Prophylaxis: Cefazolin 1g diluted in 100 mL of 0.9% Normal Saline, administered at 12:00.

08:00 - 09:00: 30 mL Mediastinal Drain: 120 mL


09:00 - 10:00: 35 mL Pleural Drain: 80 mL
