ACLS Cardiac Arrest Algorithm – Shockable Rhythm (VF/pVT)

Step Action

1 Start CPR immediately, give oxygen, attach monitor/defibrillator


2 Check rhythm → Shock if VF/pVT
3 Immediately resume CPR (2 min), establish IV/IO access
4 Check rhythm → Shock if still VF/pVT
5 Immediately resume CPR (2 min), give Epinephrine 1 mg IV/IO every 3–5 min, continue CPR
6 Check rhythm → Shock if still VF/pVT
7 Immediately resume CPR (2 min), give Amiodarone 300 mg IV/IO bolus (second dose: 150 mg)
8 Continue cycles of CPR, rhythm checks, shocks, and drug administration as indicated
9 Consider advanced airway, capnography, treat reversible causes (Hs & Ts)

Hs Ts
Hypovolemia Tension Pneumothorax
Hypoxia Tamponade (Cardiac)
Hydrogen ion (Acidosis) Toxins
Hypo-/Hyperkalemia Thrombosis (Pulmonary)
Hypothermia Thrombosis (Coronary)
