(negatively). The sample is as follows:
Minimum of two recorded events per week
Use complete sentences
Activities should be written using bullets if several happen on the same day
DATE EVENT(S)
15th October, 2024 I arrived at Isyalikira Primary School at 9:00 am and was welcomed by the head-teacher, teaching staff
and learners.
16th October, 2024 I and my fellow students started observing experienced teachers in various classes to learn the Teaching
skills they use to teach the learners.
1st November, 2024 I and my follow students attended the first professional meeting organized by the head teacher and the
mentor. The meeting focused on teaching skills and whole school issues that were done in the first two
weeks of our teaching practice.
9th November, 2024 I was injured during sporting activities such that I was unable to teach on this day.
College supervisors came to our school to supervise us. I was not observed.
I attended the professional meeting.

6 SIXTH ITEM
CLASS ATTENDANCE REGISTER
Names of learners must be written in alphabetical order with surnames first.
Names of boys should start from top to bottom and names of girls should start from bottom to top in alphabetical order. For
girls, names with the first letters of the alphabet should be at the bottom.
Leave a gap of 10 lines between the names of boys and girls. This gap is for learners that may come in on transfer from other
schools. If a learner coming in is a girl, her name has to be written on top of the names of girls, and if a learner is a boy, his
name has to be written below the names of boys. Numbering from top to bottom has to be continuous (including the gap of 10
lines) so that the last number in the register should be the number of learners in class plus 10.
The students should mark the official register
Students should construct their own register in the file and mark as well
Draw a line to mark the end of names and below write B for totals
G
Complete all details of a register.
Mark and total the daily attendance with boys separate from girls (do not find the grand total)
Use appropriate symbols for
Present (/), Absent (A), Permitted leave (L), and Sick (S).

PROGRESS RECORD
Names of all learners should be in order of alphabet as in the attendance register
Arrange subjects in alphabetical order (those the student is teaching)
The progress record should have a minimum of 2 scores per month for each subject
Each score should have a corresponding level of achievement. Two recorded scores for each month
Marks have to be expressed as percentages of the total marks
9|Page
Enter marks of major exercises or tests in the subjects you are teaching for each learner in your class
We adopt PCAR guidelines in assessment: 80% - 100% Level 4; 60% to 79% Level 3; 40% - 59% level 2 and then 0% - 39%

ASSESSMENT GUIDELINES:
1. Infant section (standards 1 -2) 100% CA
2. Junior section (standards 3 – 4); 60% CA , 40% SA
3. Senior section (standards 5 – 8); 40% CA, 60% SA
SCORE LEVEL GUIDELINES

3. 60% - 79% level 3
4. 80% - 100% level 4
SCORE CALCULATION GUIDELINES
11 | P a g e
1. Continuous assessment (CA)
Sum of all scores (expressed as percentages) divided by the number of scores
∑x where ∑ x = sum of all scores, n = number of
n
scores added.
2. Summative Assessment (SA)
Raw scores obtained by a learner in an end of Term Test expressed as a percentage
3. End of term scores (T1 or T2 or T3)
a. Infant section – mean of scores (mean of CA + SA) since it is 100% CA
b. Junior section – Sum of 60% CA and 40% SA
c. Senior section – Sum of 40% CA and 60% SA
4. End of year score
Sum of (T1 + T2 + T3) divided by 3 or
∑ (T 1, T 2 , T 3)
3
**** Place a hard paper to mark the end of administrative records and the beginning of Professional records *****
Sample calculation of learner’s performance
From the example of Aman Jennings
i. CA =
∑ x = 60+70+90+ 30+70+80 = 400
= 66.6667 = 67 (to the nearest whole number)
n 6 6
40 60
ii. T1 = 40% of CA + 60% of SA = (40% of 67) + (60% of 70) = ( × 67) + ( × 70) = (0.4 × 67)+ (0.6 × 70) =
100 100
(26.8 + 42)% = (68.8)% = 69% (to the nearest whole number)
iii. End of year: Suppose this learner obtained 72% in term 2 and 75% in term 3 both calculated using the same procdure
as in term 1, then end of year will be calculated as follows:
T 1+T 2+T 3 69+72+75 216
End of Year = = = = 72%
3 3 3

8 EIGHTH ITEM - SCHEMES AND RECORDS OF WORK
Subjects should be arranged in alphabetical order
Every scheme must have a preamble showing the Subject name e.g. Mathematics
Records of work should be filled at the end of each week or as soon as the work for that week is taught considering the
following:
Outcomes
o should show in detail what learners are able to do after learning with examples
o should be bulleted with an opening statement “Learners are able to:”
o should have one action verb for each bullet
Remarks
o should highlight success and reasons for the success, challenges faced and their causes and including the way forward
to overcome the challenges.
o Should be a summary of all the lesson evaluations (Colleges should give more practice on writing the outcomes)
9 NINTH ITEM – LESSON PLANS
Schemes of work for each subject should be followed by lesson plans for that particular subject (the first lesson plan should be
close to the records of work while the latest lesson plan should be away from the records of work- in a book format)
Lesson plans should be detailed and show use of teaching, learning and assessment resources in developmental steps.
Lesson plans should have dates and time. They should not be left with gaps where a teacher can fill when visitors supervise
him or her.
Evaluate lesson plans soon after teaching.
10 TENTH ITEM – SUPERVISION RECORD FORM
Below is the format for supervision record form
