RESOLUTION ENDORSING THE APPLICATION OF MA. PATRICIA ANGELA P. GUILLENO
AS MUNICIPAL SCHOLAR AT THE UNIVERSITY OF THE PHILIPPINES, SCHOOL OF
HEALTH SCIENCES, PALO, LEYTE, FOR DIPLOMA IN MIDWIFERY PROGRAM, AND
THEREBY AUTHORIZING THE MUNICIPAL MAYOR, HON, LUZ CHU-PONFERRADA TO
REPRESENT THE LOCAL GOVERNMENT UNIT OF BASEY, SAMAR UNDER THE RETURN
SERVICE AGREEMENT(RSA).

WHEREAS; the Local Govenment unit of Basey, Samar, recognizes the
potential of the applicant, MA. PATRICIA [Link], who has demonstrated
academic excellencein her studies;
WHEREAS; the local government unit of Basey, Samar had been granting
scholarship io qualified college students among its flagship programs;
WHEREAS; the University of the Philippines School of Health Science includes
the primary health care approach, intended especially for the underserved in the
far-flung areas;
WHEREAS: under RA 7160 particular Section 17, (b)(2)(i) thereof,"
out health services which include the implementationof
Municipalities shall carry
programs and projects on primary heath care, maternaland child care", among
others;
WHEREAS; the local government unit of Basey, Samar under the leadership
of the Municipal Mayor, Hon. Luc Chu-Ponferrada is aware of the need for a
stronger health manpower throughoutthe Basey Community: and thus, endorsed
the appicani gbove who is a deserving nominee io study at the UPM-SHS and
the availment of its scholarship program;
WHEREFORE; Upon motion of SB Member Hon. WILFREDO O. ESTORNINOs,
and jointly seconded by Hon. PERICLES L. CINCO and Hon. PERLA PERPETUA G.
PONFERRADA in session duly assembled,be it:

approve this instant Resolution:

and, resolved further to authorize Mayor, HON. LUZ CHU
the Municipal
PONFERRADA. To represent the Local Government Unit of Basey, Samar relative
to the Return Service Agreement (RSA) under the UP SHS. Resolved finaly to
forward this resolution immediately to the Municipal Mayor. Hon. LUz Chu
