1. SCOPE

Formwork is an essential part of concrete construction as it provides the mould in which an

element is cast.

Is one of the activities that most expose workers to risk of falling from heights and crushing,
giving rise to a significant number of work accidents, often with serious or fatal consequences.

There are essentially two types of formworks: traditional formwork and prefabricated formwork.

The traditional formwork is composed of pieces of wood cut and adjusted to the elements to be
concreted.

The prefabricated formwork is composed, totally or partially, by preconceived parts to form

elements, using different materials: metallic, plastic and wood (solid, plywood, laminated or
agglomerated), being normally reused.

The prefabricated formwork also has the advantage of allowing the incorporation of protective
equipment, namely guardrails and work platforms, which serve to protect workers during
concreting work. Given the diversity of equipment, it is essential that the choice and analysis be
carried out judiciously.

Given the complexity of the task, special attention will be given to the respective risks, acting
accordingly in order to minimize them.

2. PURPOSE

Quality, environment and safety are closely linked within the scope of management of the work.

In accordance with its internal policy, Conduril considers that it’s essential to fully respect the
national or other applicable legislation and standards relating to labour law, environment and
safety, hygiene and health at work.

The purpose of this Method Statement is to define the specific tasks to be performed by Conduril
relating to Formworks, ensuring that the proposed methodology for the task is fully considered
and understood, all hazards are identified, and necessary measures are taken to ensure successful

and safe execution of the Works in full accordance with the design and relevant project
specifications.

All work will be performed in order to minimize as much as possible the environmental impact and
will be performed by teams with extensive experience in works of this nature.

4. RESPONSIBILITIES

The roles and responsibilities from CONDURIL EHSS personnel are in agreement of the approved
C-ESMP.

All workers, as well as Suppliers and subcontractors will take personal responsibility for their
actions and behaviour whilst on any part of the site. They will have to follow all site requirements,
the induction and as part of their job function and have to comply with the environmental, health
and safety rules defined.

The crane truck shall be provided along with the up-to-date inspection certificate with its lifting
components such as: chain slings, slings and shackles inspected as well by authorized third party
company.

Equipment’s such as: angle grinder, electric saw shall have in its constitution the guards with all
essential safety specifications.

For safety reasons, for the connections in between air compressor and hoses or specific hoses
joints, there shall be considered the use of whipcheck safety slings or whipcheck steel cables.

All the equipment’s before being sent to the site, shall pass through the pre-inspection approval
process, where EHS Personnel from Conduril, Riportico and STE will attend the pre-inspection
approval process and as a result, only roadworthy equipment’s will be sent to site.

5.1.1 REQUIREMENT FOR PRE-INSPECTION BEFORE START ANY ACTIVITY

For the mobilization of equipment’s on site the following will be required:

2. Pre-ispection.

Before mobilization, all equipment will undergo a pre-inspection to be carried out by EHS
Personnel from Conduril, Riportico and STE.

In this inspection the pre-use checklist, for each equipment, will be fulfilled where the specific
minimum safety requirements for each equipment will be verified.

We will use the pre-use inspection templates received from STE.

Only approved equipment’s will be used on site.

5.2 MANPOWER

- 1 EHS Officer
- 10 Unskilled Labour

All Skilled personnel will have competence certificate for the position assigned.

All operators/drivers, foremen and team leaders have in-house training on how to fight fire using
the fire extinguisher. In cases of major fire, everyone will be trained on the approved emergency
preparedness and response sub-plan to know how to act.

6. EXECUTION METHOD

- Place armatures and negatives in-situ.

These must be prepared in advance at the shipyard, to allow greater speed and safety in the
assembly.

- Place the element formwork panels, and the connection between them.

These must be upright in both directions, which is done with the aid of props.

- Before installing panels, it is advisable to use a release agent (in formwork usable).

- Concrete the element with the corresponding concrete vibration. The placement of the concrete
must be done so as not to cause segregation of the concrete.

- Wait the required time for the concrete to harden and gain enough strength to support itself.
For vertical forms, this can mean waiting overnight and removing formwork the next day (not less
than 36 hours after pouring). For horizontal slabs, it can require several days. Removing formwork
prematurely could cause the surface to sag.

- Once the concrete has set, remove the formwork carefully, starting from the top.

- When removing formwork from the sides, alternate between sides to help maintain balance.

- Clean the formwork panels.

- Complete the work, with the proper conditioning of all materials and equipment used, including
PPE's and EPC's.

The minimum strength in formwork to be achieved before the loading of the concreted pieces
depends on the structure and dimension of each piece.

The former, including its supports and foundations, will be dimensioned, and constructed in such
a way that they are:

− Capable of withstanding any action to which they are subjected during construction
− Sufficiently rigid to ensure that the specified tolerances for the structure are met and that
the integrity of the structural element is not affected.

Formers shall be sufficiently strong to withstand the pressure arising from the concrete during
compaction and shall be capable of removal without undue disturbance to the concrete.

Key actions to consider include combinations of:

− Own weight of formwork, reinforcement, and concrete

− Impulse in the formwork taking into account the type of concrete
− Overloads due to construction operations (personnel, equipment, etc.) including static and
dynamic effects of concrete placement, compaction, and site traffic

− Overloads due to wind

The formwork joints must be thin enough to prevent cement paste and other fines that make up
the concrete from escaping.

Formwork made up of large pieces must be used (in order to reduce the number of concreting
phases) that have sufficient rigidity to avoid large deformations of the mold.

Formwork that is capable of absorbing significant amounts of water from the concrete or
facilitating evaporation must be adequately moistened to reduce water leakage from the concrete.

Formers shall not be removed before the concrete has sufficiently hardened and in no case less
than 36 hours after pouring.

Prior to the start of concreting operations and after concreting, inspections, according to the good
construction practices for execution of concrete structures, must encompass:

− Inspection before concreting:

o Formwork geometry
o The stability of the formwork and its foundations
o The impermeability of the formwork and its constituents
o Removal of debris (such as dust and residue of lashing wire in the section to be
concreted)
o Treatment of construction joint faces
o Removing water from the base of molds
o Preparation of the formwork surface
o Concrete openings and gutters

− Inspection after concreting

o The strength of the concrete must be estimated to verify the possibility of removing
the formwork.
o The structure should be examined to ensure that the temporary inserts have been
removed.

6.1 TRANSPORT AND STORAGE OF MATERIALS AND EQUIPMENT

In order to minimize negative impacts, the following aspects will be taken into account to
transport, storage and maintenance of materials and equipment:

- Whenever possible, all equipment and vehicle maintenance will be carried out in the
workshop provided with effluent drainage systems for treatment units
- All fuels, oils or greases will be held in containers watertight in order to be later deposited
in appropriate places
- If it is necessary to carry out maintenance of equipment, vehicles on site, that is, outside
the workshop area, Conduril will obtain approval before the start of the maintenance, and
will arrange for the temporary storage of all waste, as provided for in the previous point
- Equipment washing will be carried out in the workshop or maintenance areas
- It’s forbidden the use of rivers, streams or other water resources for washing equipment
or vehicles
- The stored materials will be properly identified and labelled with the degree of their
environmental hazard
- Conduril will ensure that its operators / drivers are informed of all procedures and
restrictions (including areas prohibited) as well as the restriction of alcohol use during
working hours work
- Materials must be properly protected to ensure safe transport between destinations.

To ensure that traffic impacts during construction are minimized, Conduril developed a
Transportation and Traffic Management Sub-Plan which will be taken into account in this activities.

6.2 WASTE CONTROL

Waste management can be understood as a series of actions that involve the steps of
environmentally appropriate collection, transport, treatment, destination, and final disposal.
These action aims to reduce construction and demolition waste disposed of in landfills and
incineration facilities by recovering, reusing, and recycling materials allowing the best use of raw
material and the reduction of aggressions to the environment.

To minimize waste with formworks, all formworks will be reused.

Special care will be taken when using release oil.
Regarding this chapter, the waste management sub-plan must be followed.

6.3 NOISE CONTROL

Noise has been identified as one of the most undesirable for neighbouring communities and even
for workers, due to the operation of numerous machines/equipment used in the execution of
construction activities generate sounds with certain characteristics, likely to annoy or disturb the
human being or cause adverse psychological and physiological effects on him.

Noise controls are used to protect workers from hazardous noise exposure. These controls likely
won’t eliminate noise on-site but will help to minimize the damage that excessive noise can cause.
Even reducing construction site noise by a few decibels can greatly improve hearing conservation.

For noise control, Conduril will keep the noise level within acceptable limits and the yard assembly
activities should, whenever possible, be limited to normal working hours.

Sensitive noise areas will be identified, such as schools, hospitals, and communities. In these
cases, Conduril will notify these elements at least three days before the start of the work.

6.4 ROAD TRAFFIC CONTROL

Conduril is responsible for dust control in its operations and activities. Therefore, in areas of
unpaved road, the soil will be moistened whenever necessary. At the end of the job adequate
drainage and compaction conditions will be replaced.

Conduril will implement what is recommended in the Transportation and Traffic Management Sub-
Plan.

6.5 RAINWATER AND WASTEWATER CONTROL

Conduril will take reasonable measures to control rainwater and its effects erosive. Whenever
erosion is detected, either at work sites or in of execution, Conduril will reintegrate those areas
and repair them.

No discharge of effluents into waterways or soil will be allowed.

6.6 CONTROL OF HISTORICAL, CULTURAL AND ETHNIC HERITAGE

If artifacts or other elements of cultural interest are discovered during the execution of the task,
the task must be interrupted, and the competent authorities must be notified. They must inspect
the site and approve the continuation of the works. Potential impacts must be identified and
mitigating measures developed appropriate according to Cultural Heritage Sub-Plan.

6.7 RELATIONSHIP WITH THE LOCAL COMMUNITY

Conduril will maintain a good relationship with the local community, so that constraints attached
to cultural, cult or ethnic activities are respected.

Activities likely to generate noise, dust or other elements disruptors will be notified and agreed
with local communities so that may only commence after due notification and consultation with
the community that can be affected.

Conduril will minimize any disturbance to adjacent communities through any or all of the following,
at a minimum, by applying the specifications relevant.

- noise pollution
- dust
- visual impact
- disruption of access

All workers and personnel involved in the project must respect the community, in this sense,
everyone involved in the project will receive a prior induction before entering the project, where
the code of conduct and rules to be followed will be presented.

Important points that will be mentioned, but not limited to:

- Respect to all local community regardless of gender, religion, origin, skin colour or physical
appearance
- Child exploitation and school dropouts
- Involvement with the community
- Exchanges of favours

Conduril commits to attend to complaints from the resident/labor population of the surroundings
and report to the inspection any accident or incident with the population, promoting and
supporting its resolution.

7. HEALTH AND SAFETY

All workers have the right to a safe and healthy work environment where have facilities with
adequate conditions for their hygiene needs, access to potable water.

Following we present the minimum aspects regarding Health, Safety, Environment and Social
requirements:

− Provide and ensure the use of the minimum PPE, such as safety shoes, reflective vest,
safety helmet, hearing protection.
− Secure the working area identifying and signalling the working area using safety signalling
and a physical barrier such as, for example, plastic jersey barrier, signal strip or cones.
− Identify and signal pedestrian path.
− Populate and guarantee the implantation of Covid-19 protocol, as per COVID-19
Management Plan.
− Thermal screening of all employees shall be done before entering to the site.
− Health, Safety and Environment induction, GBV Induction and SEA awareness shall be done
to all employees before deputing them to site.
− Awareness training on HIV/AIDS shall be conducted.
− Provision of emergency access and egress, equipment’s, and its means.
− Toolbox talk shall be conducted in daily basis.
− Daily safety task instruction shall be carried out on daily basis.
− Be aware poisonous plants, insects, snakes, animals.
− Ensure protocol with the health center closest to each work front in order to guarantee
medical assistance in the event of a more serious emergency.
− Availability of first aiders on site.
− First aid kit shall be always available at site.
− Provision of Firefighting equipment.
− Spill kits available to attend any occurrence of spillage on site.
− Drip trays available for each machinery to contain spills.
− Availability of material datasheet of all chemical or hazardous substances.
− Availability of inspection certificates for all equipment’s and machinery where applicable.

− All equipment’s shall be pre-inspected as per approval process for all machinery.
− Checklist for all equipment’s and machinery will be prepared before activity starts.
− Lifting equipment shall be inspected before and with up-to-date certificates.
− First Aid Box shall be in all the machinery and be in the vehicle.
− Display of Emergency contact details to be provided at all the working areas.
− All the working force shall follow the health and safety rules and regulations.
− Only medically fit personnel can work at the site.
− Availability of drinking water with proper water quality monitoring report as per the
applicable legal requirement.
− Tools and materials shall not be thrown or dropped. Handlings and tool bags shall be
available and used.
− Do not allow third person / local community in the working area when not properly
authorized.
− Employees shall report all incidents and accidents to key personnel as per the emergence
response procedure.
− Any machinery movement shall be guided by the presence of flagman.
− Smoking area will be designated at working place.
− Identification of the working site and with proper layout with specific areas identified, such
as: parking area, waste area, spill kit area, etc…
− Personnel shall be aware of the C-ESMPs.
− Should not allow to do any work apart from RoW.
− Always maintain the good relationship with local community.
− Waste is not allowed on site. And all the waste shall be collected from site and disposed in
the designated area.
− Chemical spills should not be allowed on site.
− Do not attempt to capture or kill snakes / wild animals / Birds / Livestock at site.
− No open fire shall be allowed at any circumstance.
− Housekeeping shall be done at all times where work is being carried out.
− Mobile toilets shall be provided for working team as per the gender.
− Do not drive the vehicle in agriculture land / wet land
− Ensure that communities are communicated with prior to the commencement of activities
and to check that all issues relating to compensation and resettlement have been
addressed.
− Borrowed soil should be collected, only from approved source after approval received from
employer.
− Provision on site of operational EHS File will be kept all the required documents.

Our workforce is mostly hired locally, and there was no need to allocate transport for personnel.
The few non-local workers will be transported in Conduril 4x4 cars assigned to the work.

All personnel will be provided with suitable Personal Protective Equipment.

The risk assessment for formworks is presented on attachment 1 and the respective mitigation
measures on attachment 2.

This risks and mitigation measures will be presented to all personnel involved on the activity,
before starting this activity.

8. ENVIRONMENTAL PRINCIPLES

Conduril will perform the work taking into account the follow objectives on environmental level:

- Continuous improvement

- Pollution prevention

- Compliance with applicable legal requirements or other requirements.

The following principles will be considered:

- The environment is considered to be composed of two components: biophysical and

social

- Construction is a disruptive activity, and special attention will be paid to the

environment, including social environment to minimize the impacts

- All norms relating to international, national, provincial and local legislation, as the
case, will be respected

- All relevant authorizations, licenses and permissions will be obtained from the
competent authorities to carry out construction activities

All waste produced during construction will be treated according to waste management plan.

There will be an eco-point for separating waste (paper, plastic, organic waste) and identified
places for the separation of waste produced on construction site (iron, wood, etc).

It is foreseen 2 bathrooms for each work front.

This dimensioning may change, depending on the number of workers allocated to each work front.

Daily cleanings will be carried out and waste collection will be carried out by certified operators.

For the hazardous waste which could result from the maintenance of the equipment, oils or inks
for example, these will be delivered to licensed operators and a copy of the license will be sent
before sending it away.

All waste will be collected and deposit, for each type, according to the min required by legislation.

Records will be kept as evidence of the proper treatment of these wastes.

Risk mitigation measures involves development of mitigation plans designed to manage,

eliminate, or reduce risk to an acceptable level and include restitution for any damage to the
environment caused by those effects through replacement, restoration, compensation or any
other means.

Environmental mitigation measures for formworks are presented on attachment 3.
