2. I submit that my Husband Late SOMAIAH, S/o. [Link] subbaiah died on:09-08-2008
at Secunderabad.
3. That I am submit that this affidavit to obtain death certificate of my Husband Late SOMAIAH.

4. I submit that the same was not to informed before the concerned Registrar of Deaths due to
family problems.

5. Hence I pray before the concerned municipal authority may be pleased to issue Death
Certificate in the light of my sworn affidavit.

6. That the facts mentioned above are true and correct to the best of my knowledge and belief.

[Link].2-22-1/114/G1, Sapthagiri Sadan Apts, Bhagya nagar colony, Opp: KPHB colony, Kukatpally,
Hyderabad. A.P. And I am the care taker and well wisher of Late ALLA NAGARATNAMMA
look after the welfare of the said deceased person and the cremation was took place at
Venkatrao nagar, Kukatpally Barial ground.

3. I submit that this affidavit is submitting to the concerned passport authorities for your kind
perusal.
