What is Primary Health Care (PHC)?

Primary Health Care (PHC) means basic health services that are available to everyone in the community at an
affordable cost. Its main goal is to help people stay healthy and receive treatment without financial problems.

PHC focuses especially on:

Women
Children
Families
Rural and poor communities

It includes:

Health education
Disease prevention
Treatment of common illnesses
Vaccination
Maternal and child care
Clean water and sanitation
Essential medicines

Doctors, nurses, pharmacists, and other health workers work together in PHC centers.

Introduction to PHC
PHC is the first level of contact between people and the health-care system. It helps people receive:

Regular health checkups

Advice about healthy living
Treatment for common diseases
Care for long-term illnesses like diabetes and hypertension

The aim is:

Better health for everyone

Equal access to care
Lower medical costs
Healthy communities

Development of PHC
India started primary health services even before the famous Alma-Ata Declaration of 1978.

Important steps included:

National Family Planning Program (1952)

Rural health missions
Community health workers

These programs helped reduce:

Maternal deaths
Child deaths
Infectious diseases

Later, WHO and many countries supported PHC worldwide.

Alma-Ata Declaration (1978)

In 1978, an international conference was held in Almaty (formerly Alma-Ata).

Main ideas:

Health is a basic human right

Every person should receive health care
Governments are responsible for providing health services
Community participation is important
Prevention is better than cure

The slogan was:

“Health for All”

This conference became a major turning point in global public health.

Sustainable Development Goals (SDGs) and PHC

The United Nations created 17 Sustainable Development Goals (SDGs) to improve life by 2030.

SDG 3 focuses on health:

Ensure healthy lives

Promote well-being for all ages
Universal Health Coverage (UHC)
Affordable and quality health care

Other SDGs like:

Clean water
Nutrition
Gender equality
Clean energy
also support PHC indirectly.

Components of Primary Health Care

There are 8 essential parts of PHC:

1. Health education
2. Proper nutrition
3. Safe drinking water and sanitation
4. Maternal and child health care
5. Immunization
6. Control of local diseases
7. Treatment of common illnesses and injuries
8. Supply of essential medicines

Pillars of Primary Health Care

PHC is built on four main pillars:

1. Community Participation
People help in planning and improving health services in their community.

2. Intersectoral Coordination
Different sectors work together:

Education
Agriculture
Housing
Food
Communication

3. Appropriate Technology
Health technology should be:

Simple
Affordable
Useful for local people

4. Support Mechanisms
Support from:

Doctors
Nurses
Community health workers
Government
NGOs

is necessary for successful PHC.

Features of PHC
PHC:

Is low-cost and effective

Promotes equality
Focuses on prevention
Provides emergency support
Improves community health
Supports social and economic development

Challenges of PHC
Some major problems are:

Shortage of doctors and nurses

Poor hospitals and equipment
Lack of money
Weak infrastructure in rural areas
Limited community participation
Poor transportation and communication

These challenges make PHC difficult in remote areas.

PHC and COVID-19

PHC played a very important role during the COVID-19 pandemic.

PHC helped through:

Early diagnosis
Vaccination
Contact tracing
Health education
Community awareness
Care for older people

Older adults were at higher risk because of weaker immunity and chronic diseases.

The World Health Organization helped countries by:

Giving health guidelines

Supporting vaccination programs
Improving emergency response systems

Astana Declaration (2018)

After 40 years, another important conference was held in Astana in 2018.

Main goals:

1. Strong political support for health

2. Sustainable PHC systems
3. Empower communities
4. Cooperation between governments and organizations

The declaration renewed the commitment to:

Universal Health Coverage and PHC for all

Sustainability in PHC
Sustainability means continuing quality health services for a long time.

To make PHC sustainable:

Governments must increase health funding

More health workers are needed
Rural health services should improve
Strong leadership and planning are necessary
Communities should participate actively

Good PHC helps countries handle:

Pandemics
Chronic diseases
Environmental problems
Aging populations

Conclusion
Primary Health Care is the foundation of a healthy society. It provides affordable, accessible, and equal health
services to all people. PHC focuses on prevention, health promotion, and community participation. It became
especially important during the COVID-19 pandemic. Strong PHC systems help improve public health, reduce
inequality, and support sustainable development worldwide.
