PARENT’S CONSENT

a Student of Cebu Technological University – Moalboal Campus, have hereunto grant permission for my son /
daughter to participate as choir member in the Sinulog Dance Competition to be held in Abellana Cebu City on
January 18, 2026.

We further give our consent for our son/daughter to travel to Cebu City on January 14, 2026 in preparation
for the said competition. It is clearly understood that the transportation to and from Cebu City shall be fully
catered and provided by the Municipality of Moalboal. We also make known our continued financial and moral
support to our son/daughter during the duration of the activity. Furthermore, we agree to abide by and adhere to
any rules, regulations, and disciplinary actions imposed by the university should our son/daughter fail to observe
proper conduct and responsibilities during the activity.

WAIVER

THIS IS TO CERTIFY that the Cebu Technological University, Moalboal Campus and the CTU
MOALBOAL MUSICAL ENSEMBLE are in no way responsible nor shall pay compensation for any accident,
harm or injury that may be caused on our son/daughter during the training, provided all precautionary and
preventive measures are being implemented to prevent any accident to happen.
It is fully known that we have read and understood all the contents on the parent’s consent and waiver and
have signed the same with our voluntary act and deed.
