I am pleased to inform you that you have been offered PROVISIONAL ADMISSION for a two-year HND
programme in the Department of MICROBIOLOGY

This offer is subject to satisfying the minimum entry requirements for the programme as approved by the
Polytechnic. You are therefore, required to produce the originals of all your credentials.

It must be emphasized that if at any time it is discovered that you made false declaration, this offer of
admission shall be withdrawn and you will be expelled. No change of name would be allowed once you are
registered in the Polytechnic except on marital grounds.

You must produce a medical certificate of fitness issued by the Director, Medical Services of the
Polytechnic.

Attached to this letter, is all information relating to financial guide and procedures for registration.
