______
A 36-year-old female underwent a course of platinum-based chemotherapy
for cancer treatment. Her pre-treatment audiometric evaluation was entirely
within normal limits. Post-treatment, she returned for a follow-up hearing
assessment. Otoscopic examination revealed normal outer ears and intact,
mobile tympanic membranes bilaterally.
Her post-chemotherapy audiogram demonstrated a newly acquired, bilateral,
symmetrical hearing loss of mild severity, isolated to the high frequencies
(specifically at 4000 Hz and 8000 Hz). Bone conduction thresholds were
identical to air conduction thresholds at all tested frequencies.
Based on the provided information, please address the following:
1. What is the most likely diagnosis?
2. Classify the type of hearing loss (e.g., conductive, sensorineural,
mixed) based on the audiometric findings.
3. Describe the primary anatomical site of the lesion.
4. Explain the expected tympanometric findings for this patient.
5. If this patient were to be monitored prospectively during treatment,
what is one sensitive objective auditory test that could detect the early
damage before it appears on a standard audiogram, and provide the
rationale for its use?
