your last appointment and whether you have noticed any changes in your mood, sleep,
appetite, energy, or daily routine. Patient: Overall, I have been doing fairly well, though I
still have some days where I feel more tired and stressed than usual. My sleep has been
inconsistent at times, especially when my mind stays busy at night, but appetite has remained
stable. Provider: Have you noticed any changes in motivation, concentration, or ability to
complete daily responsibilities? Patient: Concentration has been mostly okay, although I do
get distracted when I have a lot on my mind, but I am still managing my regular activities.
Provider: Let’s review your medications. Have you been taking them regularly as
prescribed, and have there been any missed doses or difficulty following the schedule?
Patient: I have been taking them consistently, with only one missed dose recently, but
otherwise I have stayed on track. Provider: Do you feel the medication is helping with your
symptoms, and have you noticed any side effects such as dizziness, nausea, headaches,
sedation, or restlessness? Patient: I think the medication is helping because I feel more
emotionally balanced than before, and I have not had any major side effects. Sometimes I feel
mild tiredness, but nothing severe or concerning. Provider: Any changes in how long the
medication takes to work or how effective it feels during the day? Patient: It seems to be
working steadily, and I have not noticed any major changes.
Provider: Have there been any recent stressors at home, work, or in personal relationships
that may be affecting your symptoms? Patient: Work has been somewhat stressful lately, and
there have been a few days where I felt overwhelmed, but I have been managing. Provider:
Any episodes of increased anxiety, sadness, irritability, panic symptoms, or difficulty coping
since our last visit? Patient: I still have occasional anxiety during stressful situations, but it
