GENERAL CONDITIONS

1. All bidders must submit, together with this quotation, the following eligibility documents (must be present during
opening):
a. Mayor/Business Permit;
b. BIR Certificate of Registration;
c. PhilGEPs Certificate of Registration/Membership (Red Membership is only allowed for EPC below 1 Million
Pesos);

Other required documents (must be present during post-qualification):

d. Tax Clearance;
e. Certificate of Registration: Department of Trade and Industry (DTI) for Sole Proprietorship, Securities and
Exchange Commission (SEC) for Partnerships and Corporations, or Cooperative Development Authority
(CDA) for Cooperatives as dealers/suppliers of the goods or services.

2. All bids may be delivered sealed, or sent through electronic mail to [Link]@[Link],
and addressed to the SBAC Chairperson, Mindanao Inclusive Agriculture Development Regional Field Office XI,
Regional Government Complex, Bago Oshiro, Tugbok District, Davao City;

3. All bids without price quotation shall automatically be rejected upon opening thereof;

4. All prices quoted herein are valid, binding and effective at least 90 days from the date of quotation;

5. No bids shall be accepted after the time specified for the deadline of submission of bids;

6. Bids shall be opened ONLY by the Special Bids and Awards Committee (SBAC) on the scheduled date of opening
of bids at the DA-MIADP Regional Office, or any designated room/location;

7. Award shall be made on per: (1) Item Basis, (2) Total Quoted Price, or (3) By Lot

8. All procurement shall be governed by the pertinent provisions of the World Bank New Procurement Regulations
for IPF Borrowers dated November 2020;

9. Failure on the part of the winning bidder to furnish the goods or services within the prescribed period shall make
the bidder liable to the Government liquidated damages equal to at least one-tenth of one percent (0.1%) of the
cost of the unperformed portion of every day of delay;

10. The awarding of contract to the winning bidder is subject to the approval of the Head of Procuring Entity (HoPE)
of the DA-MIADP RPCO XI;

11. The winning bidder shall submit the Notarized Omnibus Sworn Statement (OSS) prior to the issuance of Notice
of Award (NOA).

12. Failure to comply with these conditions shall mean disqualification of your bid proposal.
