12-Week Discipleship Plan
Purpose: To help a growing believer deepen their relationship with Christ, build spiritual habits,
and begin investing in others. This plan is designed for one-on-one or small group discipleship.

WEEK 1: The Call to Follow
Scripture: Luke 9:23–27
Theme: Discipleship means surrender. Following Jesus requires self-denial and daily
commitment.
Questions:
What does "take up your cross daily" mean in your life?
What might Jesus be asking you to leave behind?
Application: Identify one area where you need to surrender more fully.
WEEK 2: Who Is Jesus?
Scripture: Colossians 1:15–20
Theme: Jesus is supreme over all creation. Knowing His nature shapes how we follow.
Questions:
What do these verses reveal about Jesus?
How does this change how you relate to Him?
Application: Worship through journaling or a song reflecting on Jesus' supremacy.
WEEK 3: Identity in Christ
Scripture: 2 Corinthians 5:17, Romans 8:1–17
Theme: Our identity is in Christ, not performance or failure.
Questions:
What lies have you believed about your identity?
What truths from Scripture speak to those lies?
Application: Write a personal "In Christ, I am..." statement.
WEEK 4: The Word of God
Scripture: 2 Timothy 3:16–17, Psalm 119:9–11
Theme: The Bible is essential for growth and correction.
Questions:
What role does Scripture play in your daily life?
How can you develop consistency in reading?
Application: Practice SOAP (Scripture, Observation, Application, Prayer) with one passage this
week.
WEEK 5: Prayer and Intimacy with God
Scripture: Matthew 6:5–13, John 15:1–11
Theme: Prayer is relationship, not just request. Abide in Christ daily.
Questions:
How is your current prayer life?
What makes prayer challenging or joyful for you?
Application: Set aside 10 minutes daily this week for silent or guided prayer.
WEEK 6: The Holy Spirit
Scripture: John 14:16–17, Acts 1:8, Galatians 5:22–23
Theme: The Spirit empowers and transforms. We live Spirit-led lives.
Questions:
How aware are you of the Holy Spirit’s presence?
What fruit do you see in your life? What’s missing?
Application: Pray each morning: "Holy Spirit, lead me today."
WEEK 7: Freedom and Repentance
Scripture: James 5:16, 1 John 1:9, Romans 6:1–14
Theme: Confession brings freedom. Grace empowers change.
Questions:
What areas in your life need healing or freedom?
How can you walk in repentance, not shame?
Application: Confess a hidden sin to God (and a trusted person if ready).
WEEK 8: Spiritual Habits
Scripture: 1 Timothy 4:7–8, Hebrews 12:1–2
Theme: Discipline builds spiritual resilience. Questions:
Which spiritual habits are strong? Which are lacking?
What helps you stay consistent?
Application: Create a simple "Rule of Life" (daily/weekly spiritual rhythm).
WEEK 9: Gospel and Evangelism
Scripture: Matthew 28:18–20, Romans 10:14–17
Theme: Every disciple is sent. Share your story and His.
Questions:
What fears or barriers keep you from sharing your faith?
Who is God putting on your heart?
Application: Write and practice a 3-minute testimony.
WEEK 10: Community and Church
Scripture: Acts 2:42–47, Hebrews 10:24–25
Theme: We grow in Christ through committed community.
Questions:
What role does community play in your faith?
Are you known and growing in a local church?
Application: Take a step toward deeper community (join group, serve, etc.).
WEEK 11: Suffering and Perseverance
Scripture: Romans 5:3–5, James 1:2–4
Theme: God uses suffering to mature us. Faith deepens in trials.
Questions:
What trials are you facing? How are they shaping you?
What has God shown you through pain?
Application: Journal a prayer of surrender and hope in suffering.
WEEK 12: Becoming a Disciple-Maker
Scripture: 2 Timothy 2:2, Matthew 28:19–20
Theme: Discipleship multiplies. Your life can shape others.
Questions:
Who could you invest in?
What have you learned that you can now pass on?
Application: Identify and begin praying for someone to disciple.
