General Notes

General
1. All construction shall conform with local, county, and state D.O.T. engineering standards for
road & bridge construction.
2. The contractor shall comply with local standards and specifications for stripping, clearing,
grading, backfilling, and embankment preparation.
3. It will be the responsibility of the contractor to acquire the necessary right-of-way (ROW)
utilization permit(s) and provide for the safety of local traffic during construction.
4. The contractor shall coordinate all work within existing road ROW’s with the local traffic
engineer.
5. It will be the responsibility of the contractor to ensure that all required permits are obtained
before Beginning construction. Copies of all permits shall be provided to local authorities.
6. The contractor shall be responsible for meeting all inspection criteria and schedules, and for
signing said Inspections.
7. The contractor shall prepare and submit to the engineer a project schedule and update said
schedule on a monthly basis.
8. The contractor shall control his operations and those of his subcontractors, and all suppliers to
assure the least inconvenience to the traveling public. The contractor shall maintain free and
unobstructed movement of vehicular traffic and shall limit his operations for the safety and
convenience of the traveling public. Under all circumstances, safety shall be the most important
consideration.
9. The contractor shall be responsible for meeting all inspection criteria and schedules, and for
signing for said inspections.
10. Prior to commencing work the contractor shall furnish, erect and maintain all barricades,
warning signs, and markings for hazards and the control of traffic, in reasonable conformity with
the manual of uniform traffic control devices for street and highways or as directed by the local
traffic engineer.
11. A control line will be staked by the owner for the west property line. It shall be the contractor's
responsibility to maintain the control lines.
12. All work and all materials furnished shall be in reasonably close conformity with the lines,
grades, grading sections, cross sections, dimensions, material requirements, and testing
requirements that are specified in the contract, plans or specifications.
13. The contractor shall provide the engineer 24-hour advance notification for the following
construction and observation activities:
 Storm drainage lamping
 Pre-final observation
 Final observation
14. The engineer reserves the right to require the contractor to uncover, retest and/or perform any
action necessary to ensure that the improvements have been constructed in accordance with
the plans and specifications.
15. The contractor shall comply with all federal, state and local laws and regulations controlling
pollution of the environment.

16. The contractor shall familiarize himself with the policies and guidelines established by local
authorities for the preservation of all public and private property. The contractor shall be
responsible for all damage or injury to property of any character, during the execution of the
work, resulting from any omission, neglect, or misconduct in his manner or method of executing
the work, or at any time due to defective work or materials.
17. The engineer shall submit signed and sealed as-built plans to local authorities in accordance with
local authority standards.
18. The Engineer of Record shall provide local authorities with a statement indicating that all
conditions have been complied with regarding the approved plans, development order and/or
conditions of approval.
19. The Engineer of Record shall conduct the necessary inspections of the construction of those
facilities that will become the property of, or come under the operation of local authorities and
shall provide local authorities with a certification that the construction is in substantial
compliance with local authorities standards. Local authorities shall have the opportunity, but not
the obligation, to conduct whatever additional inspections it deems necessary. Local authorities
shall receive copies of the engineer's inspection reports, test results, copies of the engineer's
certifications of completion to the local water management district, plus the agency's
acceptance/transfer to operating permits for the stormwater management system.
20. All elevations shown hereon are based on N.G.V.D. datum.

Earthwork
1. The contractor shall submit and receive approval of the erosion control plan prior to the pre-
construction meeting.
2. It will be the responsibility of the contractor to obtain a copy of the geotechnical engineer's
report for this project and meet the geotechnical engineer's requirements for soil preparation
on the site.
3. The contractor shall coordinate all backfill operations with the resident soils engineer.
4. The contractor shall be required to keep a water truck onsite to control dust during mass
grading.
5. All fill material needed to construct the proposed roadway typical, drainage structures,
driveways, guardrail, ponds, ditch grading and maintenance of traffic shall be provided by the
contractor from areas provided by the contractor.
6. Unsuitable material may be encountered on the job. See cross sections and soil survey. For
information. Undercutting shall be required as shown in index 500.
7. All artifacts (arrowheads, pottery, etc.) Found on the site shall remain the property of the owner
and upon such discovery, the owner or his representative shall be notified immediately.
8. All excavated material not reused in the construction shall be removed and disposed of in an
area provided by the contractor.
9. In areas which require fill material the contractor will strip or otherwise remove all vegetation
such as brush, heavy sods, heavy growth of grass, decayed vegetable matter, rubbish and any
other deleterious material before embankment is started. Immediately prior to the placement
of fill materials, the entire area upon which fill is to be placed, shall be scarified in a direction
approximately parallel to the axis of fill. The geotechnical engineer shall approve the area prior
to the placement of fill.
10. Contractor is responsible for removing unsuitable material in storm drain trenches and
backfilling with suitable material.
11. Compact the top 24 inches of utility trenches within future roadways areas to 98% of the
modified proctor density; within other areas to 95%.
12. The contractor shall insure that proper soil densities are achieved for placement of all
headwall/endwall footings, retaining walls, and in general, any footing support described on
these plans. It will also be the responsibility of the contractor to ensure that sufficient soils
testing has been performed prior to construction. All organic soils encountered in building
footprint area are to be removed and replaced with suitable fill material.
13. Mass grading shall be to plus/minus 0.2 ft. and all fill shall be compacted to 95% max. density.
AASHTO T-180.
14. Contractor is to assume all soil within proposed retention basins is unclassified. Soil survey is
available for contractor information.
15. Cross sections are plotted with 12" limerock base. See typical section for allowed base options.

Drainage
1. All existing drainage structures are to be removed unless otherwise stated in the plans. All
drainage structures removed shall be disposed of by the contractor in areas provided by the
contractor.
2. Structures designated as manhole J-1 (mod.) is a type 1 inlet mirrored about centerline of index
210 to the back of inlet.
3. Contractor shall build any structures in plans designated as modified in description according to
scaled plans.
4. The existing water table is at or near the existing ground surface. The contractor is to be aware
that dewatering will be excessive and severe. Contractor is responsible for dewatering as
necessary to construct the project. The contractor is responsible for obtaining any dewatering
permit that may be required by the local water management district and meeting all
requirements of said permit.
5. Contractor is responsible for trench protection during storm drain construction/installation
including but not limited to: trench boxing, sheet pile, sheeting and shoring, utility pole support,
signal pole support or any method as approved by the engineer in order to protect workers,
utilities, structures or adjacent property and to prevent cave-in of trenches. Open trenches
within 10 ft. Of signal strain poles shall be limited to a 1/2 days operation. Open trenching within
10' of signal strain poles during raining and/or wet condition will be prohibited.
6. The contractor shall comply with the latest edition of the design standards for design,
construction, maintenance and utility operations using the following, but not limited to, the
following items:
 Index 200 - Structure bottoms Type J and P
 Index 201 - Supplementary details for manholes and inlets
 Index 210 - Curb inlet tops Type 1, 2, 3 & 4
 Index 231 - Ditch bottom inlet Type B
 Index 232 - Ditch bottom inlet Types D, D, E & H
 Index 240 - Skimmer for outlet control structures
 Index 272 - Cross drain mitered end section
 Index 273 - Side drain mitered end section
7. During construction, no direct discharge of water to downstream receiving waters will be
allowed. The contractor is responsible for maintaining water quality and shall route discharge
water in such a manner to adequately remove silt prior to runoff from site.
8. All reinforced concrete pipe (RCP) shall be minimum class III.
9. The seasonal high-water table for all ponds is 100.00.
10. Contractor shall submit to local authorities a notice of intent (NOI), notice of termination (NOT)
and stormwater pollution prevention plan that meets permit requirements.

Pavement
1. Areas within the project where curbs and/or traffic separators are being removed or modified,
the contractor shall use new construction pavement design for new pavement.
2. All paved connections shall be constructed with new construction pavement design.
3. All sweeping shall be with a vacuum type street sweeper, supplemented by hand sweeping.
4. The contractor is to be aware static rolling is required within the limits of this project.

Driveways
1. Saw cut all paved or concrete driveways at right of way or back to point of harmonization in
temporary contraction easement. Replace with like material. Use 6' concrete for concrete drives
or 6" limerock base with 2' asphalt paved driveways. Transition from new driveway width to
existing driveway width.
2. Contractor shall build a 5" compacted recycled asphalt pavement (RAP) base driveway on all
unpaved driveways to right of way line or back to point of harmonization in temporary
construction easements. RAP shall be sprayed with 0.2 gal/sy of AC-20 emulsified asphalt and
covered in clean sand and seal with roller. Match width of new driveways.
3. Driveway locations and dimensions are shown in plans. Build driveway type M 13 in urban
roadway section.

Utilities
1. The contractor shall be responsible for locating and verifying all existing utilities prior to
construction, and for notifying the various utility providers to make the necessary arrangements
for any relocation, temporary disruption of service, or clarification of activity regarding said
utility. The contractor shall exercise extreme caution when crossing an underground utility,
whether shown on these plans or field located. All utilities which interfere with the proposed
construction shall be relocated by the respective utility providers and the contractor shall
cooperate with the utility provider during relocation operations. Any delay or inconvenience by
the various utilities shall be incidental to the contract and no extra compensation will be
allowed.
2. The location of all existing utilities, storm drainage systems and topographic features shown on
the plans have been determined from the best information and are provided for the
convenience of the contractor. The engineer assumes no responsibility for their accuracy.
Should a discrepancy arise between these plans and actual field conditions, which would
appreciably affect the execution of these plans, the contractor will halt all construction and
notify the engineer immediately.
3. Utilities are to be adjusted by others as directed by the engineer. Utility owners include:

4. The location of the utilities shown in the plans are approximate only. The exact location shall be
determined by the utility company during construction. Contractor shall be required to notify
existing utilities at least 48 hours prior to beginning excavation.
5. Contractor is to be aware that existing aboveground and underground utilities are in place and
shall protect such utilities. The contractor is responsible for repairing any utilities damaged as a
result of construction activities.

Sodding
1. Sodding shall be Centipede within curb and gutter section and Bermuda for all other areas.
2. Contractor shall sod any earthed area within the project limits. This includes the utility strip,
back of sidewalk to right of way line, any median, shoulders down side streets, ditches down
side streets, ponds, and in transition areas.
3. Contractor shall warranty sod throughout the life of project. Water and fertilize as required by
the specifications. Contractor will replace dead sod as necessary until final acceptance.
4. Contractor is responsible for hand laying and pegging sod in various areas. These areas include
around headwalls, slopes steeper than 1:3, or fill height greeter than 10'. All slopes 1:2 or
steeper shall be pegged with 25% overlap.

Traffic Control
1. The maintenance of traffic for this project shall be in accordance with the Manual of Uniform
Traffic Control for Streets and Highways (U.S. Department of Transportation F.H.W.A.). Attention
is directed to index nos. 600, 622, 623, 625, 626, 627 and 660 of the Roadway and Traffic Design
Standards booklet dated January 2002.
2. The contractor shall remove or cover any signage or pavement markings which conflict with the
traffic control through the work zone, when the conflict no longer exists the contractor shall
restore the signs or pavement marking to their original position. Removal of existing pavement
markings shall be accomplished by milling, grinding, sand or hydro blasting or by any other
method approved by the project engineer. Use of paint to cover existing pavement markings is
prohibited.
3. Construction signing shall be installed prior to commencement of construction. Signing shall be
maintained during all phases of construction. Advance warning signs shall be used in accordance
with standard index 600 series, roadway and traffic design standards, January 2002.
4. The contractor shall maintain a walkable pathway for pedestrians/school children, during the
reconstruction of any existing pathways/sidewalks, as per index 660. All costs are included in the
maintenance of traffic pay item no. 102-1.
5. Whenever construction equipment is relocated and/or driven on open travel lanes, the
contractor shall utilize Index No. 627.
6. Use of traffic control officers for traffic control required when signals are not in use to maintain
traffic flow through the construction work zone.
7. The contractor shall make provisions for the removal of storm water from the roadway(s) during
construction. Provisions shall be submitted to the engineer for approval prior to use.
8. Any erosion control items not showing in the plans, but necessary under the contractor’s
sequence of construction shall be included in the related pay item.

9. Traffic control devices (i.e. temporary barrier, barricades, etc.) shall be a minimum of 2 feet from
travel lane. Variance from this criteria shall require written approval by the project engineer.
10. Traffic disruptions which are not shown on the traffic control plan, but which are necessary to
construct the project, shall be submitted in writing to the engineer and approval shall be
obtained 72 hours prior to the commencement of the work. Submittal material shall include
sketches, calculations, and other data required by the project engineer.
11. Contractor to obtain permit for drainage, overbuild and resurfacing night work from the local
permit office prior to the start of work.
12. Temporary road closure for curb and gutter construction shall be minimized. Once the daily
work is completed all equipment and barricades shall be relocated to allow for at least a
minimum of one ten-foot lane. Local authorities shall be notified 48 hours prior to any lane
closures. Only one lane closure may occur at a time.
13. Portable changeable variable message sign (VMS) should be operational for at least 2 weeks
prior to the commencement of work.
14. The contractor shall restore all lanes, eliminate all drop-offs, remove all construction equipment,
and clean up and make safe all work areas.
15. The contractor shall not mill any area which cannot be resurfaced and temporarily striped within
the same work period.
16. All thru lanes, as well as all turning lanes, should be maintained during the contract time when
construction is not underway. The work for temporary timing of the existing signalization
controllers is included in the maintenance of traffic pay item no. 102-1.
17. Before opening any lane to traffic, the contractor shall replace any obliterated or removed
pavement markings with temporary pavement markings and reflective pavement markings (i.e.,
lane lines, arrows, crosswalks, stop bars).

Hazardous Materials
1. Soil, groundwater, and petroleum contamination is known to exist within project limits. See
plans for specific locations and instructions to ensure construction support of worker safety
during construction.
2. Contamination report available for contractor information.
3. Direct inquiries to district 2 construction environmental manager at 123-456-7890.

Miscellaneous
1. Need a new project financial ID number or Accreditation.
2. Any directional arrows, pavement messages, or striping removed by the contractor’s operation
shall be replaced at the end of each day’s operation.
3. Mowing and litter removal shall be scheduled every five weeks from April 1 to December 1 and
60 - 90 days (as needed) from December 2 to March 31 and once before project receives final
acceptance.

5. Any fuel storage areas shall have prior owner's approval and appropriate measures shall be
taken to insure protection of groundwater and soil resources.
