Dear Teacher-in-Charge,

I am the parent of Chua Tian Long Jeremy, a student from Primary 1 IN.

I would like to respectfully seek your permission for my son to be dismissed early
at 1:40 pm today (9th February), as he has scheduled Speech Therapy and
Behavioral Psychotherapy appointments at NHCS - Rehab and Speech Clinic this
afternoon. I will be picking him up from the school office at 1:40 pm on the same
day.
