Process recording is a documented account of a nurse's interactions with a patient. It
includes verbatim dialogue, the nurse's observations, and reflections on the conversation.
This tool helps in analyzing and improving therapeutic communication and care.
Purpose of Process Recording
1. Enhance Therapeutic Communication: Helps nurses identify effective communication
techniques and areas for improvement.

fostering professional growth.
3. Clinical Supervision: Provides supervisors with insights into the nurse's clinical practice
for feedback and guidance.
4. Patient Understanding: Helps in understanding the patient’s perspective, needs, and
emotional state.
5. Quality of Care: By analyzing communication patterns, nurses can improve the quality of
care provided to patients.
Techniques Used in Process Recording

capture the interaction.
