Good morning again Panel of Assessor. I am now ready to perform my Conduct
Competency Assessment.
After checking the Assessment Area, I found it complies with the OHS
requirements. The tools, materials and equipment’s are check, complete, fully
function, safe and hazard free, and now I am ready to accept candidates for
assessment.

this qualification Computer Systems Servicing NC II. I have here my ID stating that I
am your accredited competency assessor.
I also have here a Letter of Appointment, appointing me to be your assessor.

Okay before we start, please stand for the prayer, Amen!
Now, allow me to acknowledge the presence of our TESDA Representative and the
AC Manager, but do not worry about their presence because they’re just here to
oversee the conduct of National assessment and to check if it is in accordance with
the standard of Competency Assessment Procedures and Methodologies of TESDA.
Prior to your assessment is I received your Application Form together with your
Self-Assessment Guide, stating here that you know and can perform the different
task in this assessment. Also, you have signed the Consent Form if you check
there is a CCTV camera, so you have signed the consent form allowing the
assessment center to cover the CCTV footage for your assessment. Do you have a

and e-signature by the processing officer of the AC, if they give the active
email because later it’s very important in receiving your National
Certificate)

Welcome to our National Assessment for CSS NC ll, so the purpose of this
assessment is for you to know that this is to determine your levels of competency.
And with corresponding assessment certification will be issued to you.

needed in this assessment in order for you to perform the task that is required to
you.

Okay, so base on the result of your assessment I observe that you have
successfully installed the operating system on your computer and so I can
say that its quality plus its working, also if you observe your work is very clean and
neat. Okay now I’m only concern by the way you perform your task, you’re
easily distracted and out of focus most of the time that’s the reason why
you finished your task late. Although you have performed your task very well
still, you’re delay, you could have done it faster if you just focused on what you
were doing. So, in the future for you to do your job effectively and
efficiently you give more attention on what you’re doing so that you can
do a better job. Do you understand?

To claim your National Certificate (NC ll) wait for the email that will be sent to you
after this assessment and visit the TESDA office located at Government Building
Imelda Park Catbalogan City there you can have you authentication and verification
