Your driving privileges are currently revoked, suspended or cancelled. In order to reinstate your
driving privileges, you must comply with the requirements listed below:
This information is based on a review of your driving record with the Tennessee Department of Safety
as of March 30, 2026 12:13 PM Central time. Any changes to your driver record after this date could
affect the requirements needed for reinstatement.
Important: Once you have submitted the above items and have been notified that your record is
cleared, you will be required to apply for a valid driver license at a driver license service center and pay
for a driver license fee. Please bring this notice and required fees to any driver service center.
To apply or reapply for a driver license in Tennessee, you will need:
• Proof that you are a U.S. citizen (certified U.S. birth certificate, U.S. passport, etc.)
• Or proof of lawful permanent residency (i-551 permanent resident card, etc.)
• Or proof of your current non-immigrant status and length of authorized stay (passport with a
valid visa & i-94, etc.)
• Two (2) proofs of identification, such as a social security card or work ID
• And two (2) proofs of residency current with in the last four (4) months, such as utility bill, W-2,
etc.
