5.0 Composition and Duties of Safety and Health Committee

The SHC of the company is responsible to plan, develop and implement OSH policies
and programs , monitor and evaluate OSH programs and investigate all aspect of the work
pertaining to the safety and health of all the workers. SHC shall be composed of the
following in compliance with the law:

(a) For establishments with less than ten workers and low risk establishments with
ten (10) to fifty (50) workers. – A SO1 shall establish an OSH committee composed of the

5.1 Duties and Responsibilities

5.1.1 Health and Safety Chairperson

To include but not limited to the following duties:

a. Reviews health and safety committee reports

b. Reviews and approve budget necessary for health and safety and emergency

5.1.2 Health and Safety Officer

To include but not limited to the following duties:

a. Serves as Secretary to the Health and Safety Committee. As such, he shall:

i. prepare minutes of meetings;

iii. notify members of the meetings; and
iv. submit to the employer a report of the activities of the committee, including
recommendations made.
b. Acts in an advisory capacity on all matters pertaining to health and safety for the
guidance
c. of the employer and the workers.
d. Conducts investigation of accidents as member of the Health and Safety Committee
and
e. submits his separate report and analysis of accidents to the employer.
f. Coordinates all health and safety training programs for the employees and employer.
g. Conducts health and safety inspection as member of the committee.
h. Maintains or helps in the maintenance of an efficient accident record system and
coordinates actions taken by supervisors to eliminate accident causes.
i. Provides assistance to government agencies in the conduct of safety and health
inspection,
j. accident investigation or any other related programs.
k. For purposes of effectiveness in a workplace where full-time safety man is required,
he shall report directly to the employer.

7.0 Safety and Health Promotion, training and education provided to workers

Training and education cannot be over emphasized as a means of learning a

healthful and safe approach to employee work effort. Knowledge of the safety rules
and how and when to function under the rules, supplemented by compliance, is
essential to safety.

g. Employees scheduled for any safety and health training shall attend such training
h. New employees will be provided orientation and training and will be furnished
information and literature covering the company health and safety policies, rules
and procedures. This training shall be provided prior the deployment of new
employees.
i. Individual job/task training will be provided to all employees. Included on this
training regulations/standards for their job
j. Supervisors shall ensure their employees are scheduled and provided this
training as required.
k. Training addressed above shall be documented in the employees’ personal
records and/or in master training record.

7.1 Work Permit System

Work permit allow work to start only after safe procedures have been defined.

a. Any contractors/external provider that will perform any kind of activities

should accomplish a work permit prior deployment on the area.
b. Type of activity should be properly identified and hazards and controls
shall be provided (Scope of work and methodology).
c. On-duty safety officer from the external provider/contractor must be
present on site in every activity of their personnel.
d. Prohibition of Personal Protective equipment and it use shall be
discussed/included in the work permit.
e. Work permit should bear a complete signatory prior performing the
activity. This should be signed by the area owner, pollution control officer,
facility personnel and safety officer.
f. Work stoppage shall be issued when activities being performed are not
specify on the scope of work/methodology and an imminent danger is

 identified. This will automatically cancel the effectivity of the work permit
and another one shall be accomplished to control the
deficiency/conformity.

Any dangerous occurrence, major accident resulting to death or permanent total disability,
shall be reported by the company to the DOLE Regional Office within twenty four (24) hours
from occurrence using the prescribed form (Work Accident / Incident Notification).

After the conduct of investigation, the company shall prepare and submit work accident
report using the prescribed form (WAIR). Moreover, other work accidents resulting to
disabling injuries such as Permanent Partial Disability and Temporary Total Disability shall
be reported to the DOLE Regional Office within 30 days after the date of occurrence of
accident using the DOLE prescribed form (WAIR).

All near misses shall be recorded and reported. A system for notification and reporting of
work accidents including near misses within the company shall be developed and reviewed
by the OSH Committee as necessary.

ENSURING PREVENTION AND CONTROL OF HEALTH-RELATED ISSUES AND
ILLNESS
