· Don’t ask for any names or Personally Identifiable Information (PII)

· Two questions that are multiple choice [MC] with at least four options

· One question that is short answer and yields one-word responses [SA]

Example: Favorite color, lucky number, etc.

· Must gather responses from at least 75 different people!
