1. Download this package and print it out.
2. Check the DS-82 form for any mistakes and sign where it says “Applicant’s Legal Signature.” Remember: You must SIGN and
DATE your DS-82 form before mailing your documents.
3. Gather these required original documents:
Your Most Recent Passport and/or Card
Note: You must send your old passport book and/or card with your application.
One Recent Passport Photo
The photo should be stapled to your application. See last page for requirements and instructions.
Name Change document (If Applicable)
Example: Marriage certificate, Divorce decree
4. Calculate Your Fees and Prepare Money Order/Check
Passport fees must be made by check (personal, certified, cashier’s, travelers) or money order (U.S. Postal, international,
currency exchange) with the applicant's full name and date of birth printed on the front and payable to "U.S. Department of
State."
Please do not send cash with your application. You can calculate your fee using the outline below:

If you want to add 1-2 Day Delivery: Add $22.05 (Paid per application for 1-2 day delivery of an issued passport book from
the Department of State to your mailing address. Only applies to mailing addresses within the United States. Not valid for
passport cards.)
After calculating the sum, make sure you add that specific amount to your Money Order/Check.
5. Insert all required documents and the signed DS-82 form in a plain envelope Remember: Use an envelope big enough to fit
the application forms without folding.
If you are paying for expedited service, make sure to write “EXPEDITE” on the outer envelope.
6. Seal the envelope and attach the included pre-paid shipping label.
7. Mail out the envelope by dropping it off at your local USPS office or any local USPS drop-off mailbox. Track your letter at

8. The U.S. Postal Service shipping labels are only valid for 28 days from the time of purchase. Please mail your packet before
those 28 days expire.

Attach one photo, 2x2 inches in size. Typically you can get it taken at your local USPS. To avoid processing delays, be sure
your photo meets all the following requirements:
Taken less than six months ago
The photo must be in color
Head must be 1-1 3/8 inches from the bottom of the chin to the top of the head
Head must face the camera directly with full face in view
No eyeglasses and head covering and no uniforms*
Printed on matte or glossy photo quality paper
Use a plain white or off-white background
Head coverings are not acceptable unless you submit a signed statement verifying that it is part of recognized,
traditional religious attire that is customarily or required to be worn continuously in public or a signed doctor's
statement verifying its daily use for medical purposes.
