Respected Sir,

I hope this letter finds you in good health.


I am Shaan M. Khan, a student of 4th year MBBS (2021 batch) at your institution.

I respectfully wish to inform you that on 1st December, I met with a road traffic accident and
sustained multiple serious injuries, which include:

- Depressed Fracture of the frontal bone


- Fracture of the nasal bone
- Hematoma
- Fractures of the 3rd and 4th metacarpal bones
- Nerve injury involving the upper limb
- Traumatic injuries to the shoulder, back, and abdomen

In addition to the above, I have been advised to undergo K-wire fixation (surgical correction) for
the fractures of the 3rd and 4th metacarpal bones, which will further require postoperative
immobilization and rehabilitation.

At present, I am under continuous medical supervision and have been advised adequate rest and
recovery, as my condition does not permit regular academic attendance. Continued physical and
neurological recovery is essential before I can safely resume clinical and academic activities.

I shall be attaching the relevant X-rays, investigation reports, surgical advice, and medical
records for your kind perusal.
In view of my medical condition, I humbly request you to grant me medical leave and kindly
consider my situation. I assure you that I shall resume college and academic responsibilities once
my condition stabilizes and I am medically declared fit.

I shall remain ever grateful to you for your kind understanding and support.
