CHILDCARE

Introduction:
Good afternoon, Ma’am. My name is _________, a candidate for Caregiving NCII. Today I am going to
demonstrate the Competency-based Skills on Child Care.

IDENTIFICATION OF MATERIALS:

HAND WASHING:
 a soap dish holder  germicidal soap
 nail brush  and hand towel

PREPARATION/STERILIZATION OF FEEDING BOTTLES:


 bottle brush  nipple brush
 liquid detergent  basin
 clean container  feeding bottle
 bottle sterilizer  2 tongs
 doctor’s prescribed formula milk  sterile gloves

VITAL SIGNS:
 a medical tray consisting of 6 medical containers:
 for cotton balls soaked in pre-boiled water
 dry cotton balls
 cotton balls with alcohol
 empty glass for soiled cottons
 a digital thermometer and holder
 forceps and holder
 cotton buds
 lubricant
 vitamins

SPONGING AND BATHING:


 5 towelettes (face, upper extremities, chest & back, lower extremities, genitals)
 2 basins
 Baby soap
 3 towels (pat dry towel, bath towel, restraining blanket)

SHAMPOOING: FINAL RINSING


 baby shampoo  a baby bath tub

DRESSING UP:
 a complete set of baby clothes  diaper
 burping towel  bib

OTHER MATERIALS:
 hamper  trash bin
 chair  2 pails
 dipper (1 clean warm water, 1 soiled water)

PROCEDURE
Handwashing (prevent spread of microorganisms, infection, and cross contamination)
 put towel on the shoulder
 first, regulate room temperature by switching off the AC and fans
 assess and prepare hands>> remove jewelries (because they can harbor microorganism) and cut long fingernails
 stand 1-2 inches away from the sink as it is considered contaminated
 turn on the faucet and regulate the flow >>> wet hands from cleanest to dirtiest
 get soap, form a lather
 using friction: paying attention to your palm, back of the palm, inter-digitals, inter-laces, and inter-locking
 using your fingertip, rub the palm of your hand in circular motion, do the same with the other
 clean 2-3 inches above your wrist and brush your nails
 rinse from cleanest to dirtiest >>> pat dry
 use hand towel to switch of the faucet >>> discard hand towel into the hamper

Preparation/sterilization of feeding bottles


(to avoid contaminating the baby’s food and eliminate harmful bacteria)
 pre-clean nipples, tits, and feeding bottle >> rinse
 set aside in a clean container
 prepare sterilizer: open and fill tank with water >>> plug in >> open
 put the feeding bottle, nipple, and tits with the tong
 close: sterilize small parts for 10-15min; feeding bottle for 25-30 minutes
 assuming it is already 15 minutes: get the tong first and set aside for a second
 use sterile tong to get the small items
 close sterilizer >>> assuming another 15 minutes passed by,
 get the feeding bottle >>> switch off sterilizer >>> wear sterile gloves
 put boiled water into the feeding bottle >>> put milk as prescribed by the doctor
 assemble tits and nipple >>> close feeding bottle tightly
 shake (without creating bubbles) and tilt >>> set aside together with the vitamins
 Remove gloves and discard into the trash bin

Vital signs (to determine baby’s overall health, plan intervention if needed before bathing, determine readiness for a
bath, and avoid hypothermia)
 Get the digital thermometer from the case
 wipe with cotton ball soaked in pre-boiled water from bulb to stem >> discard
 Wipe using dry cotton ball from the bulb to the stem; discard cotton ball
 Disinfect with cotton ball with alcohol from the bulb to the stem; discard
 Put lubricant on the cotton buds and apply onto the tip of the thermometer
 Discard cotton bud >>> Switch on and wait for ready sign
 Expose lower part of the baby and insert tip into the anus
 Remove thermometer when it beeps and restrain the baby
 Read and record the result >>> switch off
 Disinfect using cotton ball with alcohol from stem to bulb >>> discard
 Put thermometer back in the case

Sponging/Bathing (to maintain skin integrity, freshen the baby, remove irritants from the body, improve
circulation)
EYES
 Get the baby from the crib >>> Restrain baby properly
 Get cotton ball soaked in pre-boiled water and wipe the far side eye from inner canthus to outer canthus and
discard do the same with the other eye
 Repeat 2-3 times and then pat dry

FACE – towelette 1
 Put water into the basin>> check temperature (back of palm or elbow)
 Remove baby’s hoodie and discard in the hamper
 Get 1st towelette: wet – squeeze – make a mitt
 Start from the far side: in circular motion, wipe from the forehead, nose bridge, cheeks, chin, ears, and back of the
ears
 wet – squeeze – make a mitt
 do the same on the near side face
 Repeat 2-3 times then pat dry >>> Discard towelette into the basin

UPPER EXTREMITIES (towelette 2)
 expose far side arm of the baby
 Remove the mitten and undo the dress; discard mitten into the hamper >> restrain
 Discard water; apply warm water >> check temperature
 Get the 2nd towelette: wet – squeeze – make a mitt
 Expose arm >>> wipe palm – back of the palm – between fingers
 Wipe using 4 long strokes 1,2,3,4 up to the armpit >>> Restrain
 Apply soap: wet – squeeze – make a mitt
 Expose: Wipe palm – back of the palm – between fingers, 4 long strokes 1,2,3,4 up to the armpit
 Restrain
 Replace water, apply warm water: check the temperature: wet – squeeze – make a mitt
 Expose arm: wipe from the palm – back of the palm – between fingers, using 4 long strokes 1,2,3,4 up to the
armpit
 Repeat 2-3 times and on the 3rd time,
 Replace water, apply warm water: check the temperature: wet – squeeze – make a mitt
 Expose arm: wipe from the palm – back of the palm – between fingers, using 4 long strokes 1,2,3,4 up to the
armpit >>> pat dry >>> restrain
 Do the same on the near arm side
 Discard towelette into the basin

CHEST OF THE BABY – towelette 3


 Insert the sleeves of the shirt between the arms >>> Restrain the baby
 Discard water, apply warm water: check the temperature
 Get the 3rd towelette: wet – squeeze – make a mitt
 Expose the chest of the baby: wipe from the neck, chest, pay attention to skin folds, down to the navel in circular
motion >>> Restrain
 Apply soap: wet - squeeze – make a mitt
 Expose: wipe from the neck, chest, pay attention to skin folds, down to the navel in circular motion >>> Restrain
 Discard water, apply warm water: check the temperature: wet – squeeze – make a mitt
 Expose: wipe from the neck, chest, pay attention to skin folds, down to the navel in circular motion
 Repeat 2 – 3 times and on the 3rd
 Discard water, apply warm water: check the temperature: wet – squeeze – make a mitt
 Wipe the neck, chest, pay attention to skin folds, down to the navel in circular motion >>> Pat dry

BACK OF THE BABY – towelette 3


 Remove dress completely and discard into the hamper >>> Restrain baby
 Discard water, Apply warm water: check the temperature
 Using the same towel: wet – squeeze – make a mitt
 Turn baby on the side: wipe from nape down to the buttocks in circular motion >>> Restrain
 Apply soap: wet - squeeze – make a mitt
 Turn baby on the side: wipe from nape down to the buttocks in circular motion >>> Restrain
 Discard water, apply warm water: check the temperature: wet – squeeze – make a mitt
 Turn baby on the side: wipe from the nape down to the back, and buttocks
 Repeat 2-3 times and on the 3rd
 Discard water, apply warm water: check the temperature: wet – squeeze – make a mitt
 Turn baby on the side: wipe from the nape down to the back, and buttocks
 pat dry >>> discard towelette into the basin

LOWER EXTREMITIES (far side leg)– towelette 4


 Expose baby’s far side leg and remove booties; discard into the hamper >>> Restrain
 Discard water, apply warm water: check the temperature
 Get the 4th towelette: wet – squeeze – make a mitt
 Expose far side leg: wipe from foot – foot’s sole – between the toes – using 4 long strokes 1,2,3,4 up to the groin
>>> Restrain
 Apply soap: wet - squeeze – make a mitt
 Expose leg: wipe from the foot – foot’s sole – between the toes – 4 long stroke up to the groin >>> Restrain
 Discard water, apply warm water: check the temperature: wet – squeeze – make a mitt
 Expose far side leg: wipe from the foot – foot’s sole – between the toes – 4 long strokes up to the groin
 Repeat 2-3 times and on the 3rd,
 Discard water, apply warm water: check the temperature: wet – squeeze – make a mitt
 Expose far side leg: wipe from the foot – foot’s sole – between the toes – 4 long strokes up to the groin
 pat dry >>> Restrain
 Do the same on the near side leg
 Discard towelette into the basin

GENITALS – towel 5
 Expose the lower part of the baby >>> undo the diaper
 If there is poop, use cotton ball soaked in pre-boiled water to wipe the anus until the poop is gone
 Insert the soiled cotton ball in the diaper, roll and discard into the trash bin >>> Restrain
 Discard water, apply warm water: check the temperature
 Get the 5th towelette: wet – squeeze – make a mitt
 Sponge genitals of the baby without soap >>> Wipe from front to back 2-3 times
 after the 3rd time: wet – squeeze – make a mitt
 wipe from front to back down to the anus then pat dry >>> restrain
 if the baby is a boy: discard water, apply warm water: check the temperature
 Get the 5th towelette: wet – squeeze – make a mitt
 Expose genitals: retract the skin of the penis – wipe from tip – shaft – scrotum
 Repeat 2-3 times
 after the 3rd time: Wet – Squeeze – Make a Mitt
 retract the skin of the penis: wipe from tip – shaft – scrotum down to the anus
 discard towelette >>> pat dry >>> Restrain properly
 Remove pat dry towel and discard into the hamper
 DISCARD TOWELETTTES IN PAIL WITH SOILED WATER
Shampooing (maintain hygiene, promote hair development)
 Discard soiled towelettes into the pail with soiled water
 Discard water – Apply warm water on both basins: check the temperature
 carry the baby in football hold
 Expose the head above the basin, wet, apply diluted shampoo
 Massage scalp gently to remove cradle cap and rinse on another basin
 Put the baby back on flat surface and pat dry head with bath towel
 Restrain

Final Rinsing
 Put warm water into the tub: check the temperature
 Expose the baby >>> Discard soiled restraining blanket
 Carry baby into the tub (sitting position) >>> Rinse body thoroughly
 Lay baby back and pat dry >>> Discard bath towel >>> restrain

Cleaning the Umbilicus (to prevent infection and promote healthy healing)
 Expose umbilicus >>> Get cotton ball with alcohol
 Using aseptic technique: clean from cleanest to dirtiest
(circular motion from outside going in)
 Discard cotton ball

Dressing Up (for baby’s comfort, warmth, and protection)


 Last to remove, first to put on
 Put on diaper – bonnet – dress (from the far side) – mittens – socks
 RESTRAIN neatly >>> Put on the bib

Administration of Vitamins and Formula Milk


(elevate head to prevent choking; check temperature of milk to prevent burns)
 get milk and vitamins
 Sit and cuddle the baby: elevate the head
 Get the vitamins: check expiration date
 Feed the baby with vitamins: wipe lip with bib
 Put towel on the shoulder >>> Get feeding bottle: shake and tilt >>> check temperature
 Feed the baby half way and let the baby burp
 Finish feeding the remaining milk and let the baby burp again
 Put the burping towel into the hamper
 Sing a lullaby >>> when the baby is asleep, put baby into the crib
 Remove the bib and discard it into the hamper

Do after care: collect soiled linens; collect materials and return them to their proper place
Clean and sanitize the area
