VERBATIM REPORT

Counsellor : Hi, welcome. Let’s start our session today.

Client : Okay ( the client was a bit tense )

Counsellor : I would like to assure you that whatever discussed in this session will be kept
confidential and will not be shared with anyone without your permission.

Client : Okay.

Counsellor : How have you been feeling ?

Client : I’ve been feeling very tired and sleepy and stressed.

Counsellor : How do you usually come to college ?

Client : By bus. It takes around 1 to 1 and half hours to reach the college.

Counsellor : Did you notice when did you to feel stressed out?

Client : I’m like this for the past few days.

Counsellor : Did something happen ?

Client : The exam is approaching soon. I feel very stressed about that. (looks worried )

Counsellor : That’s okay. I normal to experience stress during the exam. Is there anything
particularly you are worried about ? ( lean forward )

Client : Lately, I’m struggling to memorize what I have studied. I just keep forgetting.

Counsellor : Hmm (nods). Can you tell me how you deal with stress usually ?

Client : Okay. I usually discuss it with my mom.

Counsellor : Can you elaborate your relationship with your family ?

Client : I live with my parents. I have an elder sister.

Counsellor : How’s your relationship with your sister ?

Client : It is good. But I don’t share much with her since we have seven years gap.
Counsellor : How is your relationship with your parents ?

Client : My mom is more like a friend to me. we talk openly with each other. I can share
anything with her.

Counsellor : That’s great. And how is your relationship with your dad ?

Client : We are not that close. I speak to him only when I need something.

Counsellor : Ok. Earlier, we were talking about stress. How do you handle it during the
exams ?

Client : I usually divide into small parts and work on it. Sometimes, I ask suggestions from
my mother.

Counsellor : Oh. That’s really a good coping mechanism. What about your body pain ? can
you explain in detail ?

Client : Yes. I have been experiencing stomach pain.

Counsellor : How long are you suffering from it ?

Client : I have it since 2nd grade. The doctor said it was a gastric problem.

Counsellor : are you under any medication for that ?

Client : Yes I was given syrup for that.

Counsellor : Okay. What are your goals for our sessions together ?

Client : I want to improve my memory and my overall health.

Counsellor : Those are really important goals. Since there might be physical and mental
factors involved, how about we break it down into smaller steps?

Client : Ok. I think I can do that.

Counsellor : Here are a few things we could try:

Using memory aids – Writing things down, setting reminders, or using a planner to
track important tasks.

Mindfulness techniques – Practicing focus exercises to help train your brain to stay
present in conversations.
Lifestyle adjustments – Making sure you’re getting enough sleep, staying hydrated, and
eating brain-boosting foods.

Mental exercises – Simple activities like puzzles, reading, or even repeating information
out loud to reinforce memory.

Client : Hmm… I think writing things down could help. And maybe the mindfulness thing—
I do tend to zone out a lot.

Counsellor : That’s a great start. How about we try those for a week and check back in next
session to see what’s working for you?

Client : Yeah… I can do that.

Counsellor : Perfect. You’re taking an important step in improving this, and we’ll work
through it together. In our next session, we’ll check in on what’s working and adjust as
needed. Until then, take things one step at a time, and don’t hesitate to reach out if you need
extra support. You’re doing great.

Client : Thankyou.
