Health Check Up Silver Package Details

Process Flow – Health Check Online Purchase

1. Employee can enroll for the health checkup program between the 16th of the month to
15th of the next month.

2. Any inputs provided in the system post-15th of the month shall be processed between the
16th to 22nd of the subsequent month.

3. Connect and Heal Primary Care Pvt Ltd. will send a mailer to all employees who enrolled
between the 16th to 22nd of the month. The mailer will consist of all details on how to
book an appointment for the health check-up.

4. Employees can follow the steps and book the health checkup appointment as per the
process mentioned in the mailer.

5. Once the employee books an appointment, an appointment- generated email will be sent.
After the appointment is confirmed, the employee will receive another email mentioning
the date, time, center details, and fasting instructions.

6. In case the employee is unable to attend the medicals on the appointed date, or needs any
assistance, you can call 91118-91118 or email [Link]@[Link].

7. The employee will visit the diagnostic center with a copy of the confirmation mail and ID
card.

8. After the health check is done, the employee will receive the reports on CNH care APP.

9. The purchase is valid for one year.

10. 12 hours of fasting is mandatory before attending the medical tests.

11. The entire health package must be utilized in a single visit.

12. The components of the package cannot be split between two or more people.

13. Rescheduling of appointment will be done once.
